Looks like sausage. Doesn't cause her pain. Slightly seeping blood. Scabs up but she keeps scratching it off. Haven't seen any photos quite like it online.

That looks very angry and infected. Please get her to the vet.
If it isn't causing her pain, it could be dead tissue. It most certainly looks very painful and infected. Is she indoor only?
As an aside why is she wearing two collars? Is one a flea collar? That could be causing it.

Outdoor and indoor. One is flea collar.
Maybe try taking off the flea collar and using revolution or advantage. Flea collars are known to cause skin problems.
And definatly get her to the vet.

I agree that out of an abundance of caution you remove the flea collar. A vet needs to see this though as it is not healing if it is seeping blood. The fact that she scratches it is also an indication of discomfort, even if the scratching is causing some of the blood.
