I would either go back to your vet and ask to be referred to a specialist or change your vet If there is a new cat in your area and your cat goes in the garden then this can
be a problem my friend at work has a problem with her cat at the moment Her cat goes outside in the garden and the neighbour has a cat that will fight with other cats and it keeps comming in her garden
this is stressing her cat out badly as he is very nervous and has had problems with Cystitis with him passing blood stained urine all over the house she took him to the vets and ended up with various
medication which have now given him severe constipation which the vet did warn her about she was going to take him back to the vet again so I will find out how she got on when I see her on Monday
