Torn Claws

My cat was missing for a few days (extremely odd of her) she's an indoor/outdoor cat. She was originally feral, but she's been with us for 12 years. She comes in every single night. She finally came home not hungry and smelling of wet cat food which we don't feed her. She has a scratch on her nose, and a bunch of her claws are torn up like she was clawing her way out of something (other than that she seems like someone was taking car of her). I'm not sure what happened, but she acting odd, hasn't gone outside in 48 hours other than once just to go the bathroom, sleeping under the bed all day (which is not normal) Her nails are bloody, but they're ripped almost all the way down to the bed. Should we be concerned enough to take her to the vet?

I'm no cat expert but if your cat keeps up this behavior and continue to have these symptoms then I would definitely consider going to make sure nothing is wrong enternally.
