The pill shooter was the only thing that worked for me. And if you're using the knee method, you really do have to hold the cat down and make sure that he can't move.
I agree that asking your vet for a demonstration might help. Or there are some very helpful videos on the Cornell Feline Health Center site that are probably just as good (it's a great health site overall). Videos on the various pill giving methods are here:
