Lactulose and hypercalcemia?

Does anyone find it interesting that all these kitties had both hyperCa and GI issues?  Hazel hasn't been on lactulose in two years but we continue to fight her elevated Ca with a biphosphonate, a diuretic, a steroid, and fluids.  I'm so sick of this "no known cause" hypercalcemia that is going to steal my baby.  I wish someone would find the cause.  I wish I could find the cause!
Do you mind if I ask how old your kitty is?  What food is being fed? Is IBD a factor?  I still wonder about high phosphorus foods such as Fancy Feast, and how that might affect hypercalcemia. I lost my beautiful Tiger to pancreatic cancer in July 2015. She was stage 2 CKD, and very early stages of IBD(according to her ultrasound) and was also diabetic.  She was  17,  prone to constipation, and bordered on the high side of normal calcium level.  Prior to her diabetes and CKD diagnosis, I stupidly fed her dry food...  My vet said we could check her ionised calcium, but I declined.  I wish I had not declined that test.

Thank you for the reply.  I am so sorry to hear about your Tiger.

Hazel is about 11 now.  She was about 9 when she was diagnosed with hypercalcemia.  She doesn't have CKD.  She doesn't have IBD, either, but she has had chronic constipation for about five years, which is managed with wet food, Miralax, fiber, Cisapride, fluids, acupuncture, and Chinese herbs (specifically MaZiRen).  No slippery elm.   She always eats high-quality foods, usually Earthborn Holistic or Weruva. She did go on a freeze-dried raw kick, and was eating Stella & Chewy's and Primal for a while in recent months.  Now that she's on the appetite stimulant, I have more control over what I can give her.  In my research, I came across two reports of hypercalcemic cats being cured by eating Wellness. So now I am trying that.
