Is this stray cat pregnant or having a health issue?

I’ve been feeding a stray cat for a few months. She is so sweet and friendly. I suppose she is about 1 year old now. I think her nipples don’t look normal, is it a sign of pregnancy or a problem? (Her appetite has increased also.)
