Help ! His loosing fur on his ear and spreading

My 2 year old cat is loosing his fur on his ear and it is spreading to his face. It stared as a small bald spot in his ear but it started spreading. It's been 3 months now.The vet gave an antibacterial and antifungal lotion but it is of no use.
Please help

Did he do a woods lamp test to actually see if it was ringworm? Has he done a scraping to look for any cells or parasites? How long has she been on the topical medication?

I also wondered if a skin scraping was done. The vet may have started at the most common level of treatment but since this is continuing it might need to proceed to higher level diagnostics.

Possible food or environmental allergy?

If the current treatment is not helping and the vet has no idea what is wrong and / or unwilling to find out the cause, seek out another vet.

Update
His ears got better,he had a fungal infection.
