Going to bathroom in the bathroom and not litterbox

Well today is the 2nd time we have noticed that Oscar has went went #2 in the bathroom floor instead of in his litterbox. He did it a few weeks ago in the same room. I have shown him his litterbox again after the first time.

Any ideas on why?

Also, I have noticed that sometimes Oscar shakes while he is asleep like he is having a seizure.

Ideas on why?

Has he been to a vet?

He may have a bladder infection, that can cause a loss of control. Is he new to your house? He may be uneasy about that and going in inappropriate places in an effort to "claim" your home.

We have had him for almost 4 years. I think it is possible that the original spot for the litter box was hard for him to get to. Since I have moved it back out into the open he seems to be going to the bathroom again regularly.
