I'm not a behavioral expert by any means but when my boys were dangerously sick I read a lot about this. Basically it is cat's nature not to want to show pain. Their society in the wild is based on demonstrating dominance, so by nature they don't want to be seen when weak and vulnerable. Both my boys, young and pampered as they were, snuck off somewhere secluded at the end.
