Vomiting and BM's from seizures?

About 9 months ago, our 9 year old male cat "Sam" would vomit and poop in various places around the house. He is an only cat, btw. At first, we would come home or get up in the morning to find vomit (mostly partially digested food) and then feces close by. My first thought was that he was vomiting so hard that he would have a spontaneous bowel movement. This would happen about once a month or so.
More recently, we would find vomit close to his litterbox and then a "trail" of feces leading into adjacent rooms. After about 4 or 5 of these occurances, I took Sam to the vet. The vet's initial exam didn't reveal any obvious health problems, but rather suggested that he was possibly have seizures. He told us to just keep an eye on him for awhile.
The frequency of the vomit near the litterbox and pooping around the house episodes increased to about 3-4 /month. One of the times I happened to see Sam in his litterbox apparently trying to have a bowel movement, but was having difficulty. In fact he looked like the whole back end of his body was convulsing as if he was straining to poop. He didn't have BM, but as he exited the litterbox, he vomited right where we have found vomit many times before. He then started walking toward the adjacent room, but I shut the door to keep him in the room where his litterbox is. He seemed like he was in a little distress, but not too bad. He then turned around and went back to his litterbox and again tried in vain to have a BM. When he got out, I decided to clean the box as it had some older waste in it. By the time I finished, he had pooped on the floor on the other side of the room. He then went back to his litterbox, but again, nothing but false activity. I cleaned up his "accident" and went to dispose of it in the garage and by the time I returned, he had defecated right where he was lying on the floor! That was the end of that "episode", which actually lasted about 45 mins. He seemed more himself after that.
I took him back to the vet and they put him on Phenobarbitol and he has had at least one of these episodes per week since. The vet checked his blood for common issuses and took x-rays to look for signs of constipation. Three days ago we started giving him Laxatone to see if he needed help with his BM's, but just today he had another episode that lasted over 30 mins! Luckily this time he didn't poop on the floor, but he must have made at least 20 trips into his litterbox to try and poop. At one point only diarrhea-like liquid would come out and then finally he did have a BM, which again seemed to end the episode.
Does any of this sound familiar to anyone? Sam otherwise is healthy and a good kitty.
I forgot to mention that whereas the Vet has diagnosed these episodes as seizures, Sam doesn't exhibit any of the classic seizure symptoms during them such as paddilng, twitching, thrashing, falling over, etc. His sometimes slightly disoriented, but not always.

We're having a problem with our cat and rather mysterious vomiting. I am by NO MEANS a vet, but based on what I've learned with what's happening with Lazlo, it sounds like your kitty has a serious consitapation problem.

Did he have x-rays done? He's not developing megacolon?

Vomiting from straining is common, actually, and watery poop does not mean he's not constipated - it can mean some poop was blocking the passage.

Let me find a link I just read this evening that might help you understand. But basically everything you describe sounds like a seriously constipated kitty.

BTW, did your vet discuss with you the issues related to phenobarbitol?

I need to go find a thread for you - Snickerdoodle has a kitty that's been having a similar kind of problem that she suspected is also related to constipation rather than seizures. If I were you, I'd get an opinion from a 2nd vet at a different practice to confirm that phenobarb is the best option here.

While her vet has provided a diagnosis of feline hyperesthesia (sp?) syndrome, and that first thread is long and a lot of pages, I think you'll find it really informative based on the information in your post and given that your kitty is now on phenobarb.

Thanks LDG. The vet did do an x-ray. All looked ok. The frequency of Sam's BM's have vastly decreased. From 1-2 per day to 2-3 per week.
Another puzzling thing is that when he finally does have one, they aren't dry and hard as one would expect from being constipated.

As for the phenobarb, he was on it for 3 weeks before the vet checked his level. The range for cats is 15-45 (whatever units) and Sam was at 37.8. The vet said that even though his level was within the acceptable range, she would like to see a slightly lower number, so she decreased the AM dosage. I was happy to hear that because poor Sam was not dealing with the meds so well. He always slept a lot, but on the initial dosage he basically sleeps all day and night. He won't play and when awake, he's pretty "out of it". The reduced dosage is helping with his alertness and energy level however. He's more himself the past few days.

This is very frustrating to us because aside from the almost weekly episodes of him vomiting and then pooing all over the house, we are scheduled to go on vacation in a little over a month. I can't stand the thought of poor, sweet Sam spending 8 days in a pet kennel or similar; nor would it be fair for a realtive or friend to have to come over to find the result of one of Sam's "episodes".

I don't understand why your vet prescribed phenobarbital for your cat? This sounds like a megacolon issue, or some sort of blockage to me. I know you said x rays were done but..... A cat should be pooping more than three times a week.

Poop does not have to be hard and dry to indicate constipation. Constipation means not going poop often enough, larger than normal bowel movements are a sign of constipation in cats (and megacolon) frequently straining to poop, pooping small amounts throughout the day, all are signs of constipation and/or megacolon.

I recommend a second opinion with a different vet.

PS as for phenobarbital, sedation is normal, it takes a few weeks, or even a month or so, then their bodies adjust and they get back to their normal selves.

Please keep us posted on Sam.

I called Sam's Vet on Monday to discuss his most recent episode and to emphasize that his BM's have decreased to less than 3/week, but he never returned my call. I was planning on giving him another chance to change his diagnosis, but oh well. I then called a different vet today who got Sam in right away. As I described the situation (which was very similar to my OP), she immediately suggested that he is constipated or has some form of blockage.
X-rays revealed that his colon was packed full of feces! They recommended that he spend the night as they were going to give hime 2 enemas and a follow up x-ray in the morning. This will be followed up with a diet change and a Laxative.
By no means is he out of the woods as we still don't know why he gets blocked up, but at least we are heading down a different path. If this pans out, we will wean him off the phenobarb and not go back to our old vet!

Thank you for keeping us posted on Sam. Poor baby, he must have felt just terrible. I'm so glad you got a second opinion, Sam is lucky you take such good care of him.

I bet it feels empty in the house without him. I hope he can come home tomorrow. I will be watching for updates.

Sam is home and glad to be! He ended up getting 3 enemas and still didn't pass everything, but the vet thinks between that, the Lactulose and his new high fiber diet, he should be "moving" with no problem now.
Thank you LDG and otto for your advice and encouragment. I will post an update in a few days.

I bet he's glad to be home!!!!!!!!!!!!!!!!!!!!!!!!!!!!! It's just so stressful when they have to stay at the vet - for them and for us.

Well, it's Saturday and Sam hasn't had a BM since he came home on Wednesday. I'm getting a little concerned. He has been on his prescription diet and getting 2 doses of Lactulose daily, but nothing's coming out! He's pretty lethargic which I would expect. If he hasn't done his duty in the next day or two, I'll be taking him back to see the vet.

Take him now, today. Four days is much too long. Please don't wait any longer.

It sounds like he has a megacolon, lactulose is not the answer for that. Cisapride is. Actually at his age, surgery is the answer.

But please get him seen today, the poor boy.

I called Sam's vet on Saturday and she told us to increase his Lactulose dosage and if he hasn't had a BM by Monday to bring him in. As of Monday AM, no BM. He's back there now and will be spending 2 days there. They started an IV to build up his fluids and then did an enema about 8 hours later. I'll save the gory details, but let's just say he was very productive! They will give him another one Tuesday AM and then do another x-ray. He will then be started on Cisapride.
They don't suspect megacolon, but can't rule out him developing it down the road although the lactualose and Cisapride "should" prevent it.

I'm so sorry he's having to suffer such indignities - I really hope this will be the last of it and once he's back home again the addition of the cisapride does the job!

Sam came home after 2 days at the vet. He is now on Cisapride and Lactulose. He had 3 enemas which were very beneficial and had an IV in while there. He is his "old self" again, but time will tell if the Cisapride will help.
And BTW, he was all the vet techs "favorite" while there. No surprise here.

Sam has been home for 1 week and also has been 1 week since starting the Cisapride. It took until Saturday (3 days) for him to have a BM. He didn't have another one until Monday, but has had at least one per day since. Phew! I never thought I'd be happy to clean poop out of a litterbox!!! He seems very much himself again too.
