pooping and throwing up in the car

so my cat he is a medium size grey cat with white paws and today like an hour ago he pooped in the car and threw up and this clear sticky stuff with blood in it to but there was a litter box and he pooped in there but then he pooped everywhere and threw up a lot and he has never done that before and i have had in since he was a couple weeks old weeks old and it was really strange so can someone tell me what this means please
