Persistent Eye Issue

Hello,

My cat Jack is about 4 years old and had his right eye enucleated due to a massive infection when we found him. His left eye had corneal scarring from the herpesvirus infection as well. He always has some bouts of URI and eye infections- mainly conjunctivitis, but it usually clears up with some topical ointment and additional medication.

However, for the past 40+ days, I have been dealing with this particular eye issue. On 11/23 he was diagnosed with having a corneal ulcer and keratitis, along with conjunctivitis. The Ulcer was treated first with antibiotics and topical drops. When returning, his ulcer had healed, but the keratitis was still not clearing up. So my doctor stated it was likely an immune response, and that we should try a cycle of topical steroids. Upon returning there was little change to the state in his eye, so she assumed that he was having a viral flare-up and gave him a 7-day script of 250 mg of Famciclovir for an anti-viral and Tobradex.

After the run of the cycle, little change had occurred and it looked as the vascularization in his eye worsened. I was referred to a specialist. Jack's diagnosis on 2/2 by the specialist was that she suspected the original diagnosis being the herpesvirus. She ran several tests and determined the following:
Mild Conjunctivitis, Chronis Keratitis (mild), anterior synechiae (little impact). She stated that the anterior synechiae seemed to have little cause for concern as he did well in the vision test and that he did not appear to be squinting or having too much pressure or swelling causing pain.

She prescribed me with a 14-day script of 250mg of Famciclovir (twice daily) and a 3-week script of topical Terramycin ointment to be used 3 times a day. At this time, Jack appears to be experiencing some discomfort in his eye. Not holding it closed, but squinting here or there. His conjunctiva has swelled a bit more as well. This morning I caught him vomiting. That is the first I saw of that as when I was feeding him I gave him his Famciclovir (which is directed to take with food) and he got spooked and darted from his food bowl. I feel the drug may have dissolved and upset his stomach. Last night he also had a small bout of coughs, so I think he is showing URI symptoms as well which again, could be caused by the herpesvirus.

I attempted to call my primary and they are unfortunately closed due to a covid outbreak in their office, and the specialist's office is closed until Monday. I am giving both a call again first thing Monday.

I guess the question I have, is has anyone experience this long of a battle with an eye infection? The poor boy already has enough going on and I do not want anything to happen to his eye. It is still cloudy with the keratitis and his conjuctiva is a bit more inflammed.

Please let me know if you've had a similar situation and what worked best? Is this a normal ordeal dealing with a herpesvirus flare-up?

I have not heard of chronis keratitus. I have had experience with Eosiliphinic Keratitus which is usually linked to herpisvirus and ulcers. Did your vet mention that?

Sorry, not chronis- chronic. And yes, that is partially the issue with him.

If it's Eosiliphinic Keratitus ask your vet about 1% cyclosporine drops, they were mixed at the eye specialist. My cat was on steroid drops and antibiotic drops for months but she kept having flares, once I put her on cyclosporine it cleared up in weeks. She had to have one drop daily to keep it clear.

I really feel for you, I had a cat that fought infection for 6 months, it seemed endless. the only thing that cleared it up was teh Terramycin. it's much easier to administer if you pull dpown gently on teh lower lid and put a small ribbon there of the med in the 'pouch' that is formed. Blinking will smear it across the eye. i also wiped teh eye several times a day with a boric acid solution and sterile water (boiling) 200ml (approx. 3/4 cup) with 1 Tbs. of boric acid. Cool to lukewarm and store with a lid. Ask your vet first.
