He is almost 14 years old. This has been going on for about 3 years. He rarely coughs... except after he hisses. He hisses regularly... such as when he's on my lap, and I need to get up. A couple of years ago I took him to a vet, but he couldn't find anything wrong with him after doing tests. Does anyone have any thoughts on it? It bothers me so much that I don't even want him on my lap anymore if it's going to cause him to hiss when I get up..

Have you had him checked for this lately. You said they checked him out a couple years ago but maybe now things have changed. Is the coughing BAD when this happens? Coughing can be a sign of heart issues, in case you aren't aware. How or IF heart disease relates to hissing I have no idea, but I guess it might be possible. Maybe you could get a video of him doing it to show the Vet. And ask them to check his heart. There is even a simple blood test they can do to check it.
