Help! What’s causing this acid reflux & nausea?

Hey guys, first time poster on here. Been having a rather stressful week with my kitty Casper. He’s a 15 year old bengal. About 2 weeks ago I heard him howling in pain and a strange meow I’ve never heard from him before. I rushed to him and saw that he was struggling to throw up and eventually threw up/possibly regurgitated his food. He’s thrown up throughout his life occasionally due to eating too fast but the meows were alarming. I was hoping it was just a one time thing but unfortunately it happened again a few days later (again after eating) which I realized he was also foaming at the mouth and his vomit was foamy. I booked a vet appointment for him which was exactly a week ago today. They took a blood test which revealed that he has stage 2 kidney disease, he had no urine in his bladder so couldn’t do a urine analysis. The vet sent me home with some renal kibble and Cerenia.

Immediately after the vet he ate some of the new kibble and a few minutes later he was painfully struggling to throw up again. At this point I was pretty certain he was experiencing bad acid reflux. I did some research and bought him a raised food bowl, as well as a cat fountain which he immediately loved. I started to feed him kibble mashed up with water in small amounts every few hours and it seemed to help a lot. He was still acting normal otherwise, peeing, pooping, walking around and cuddling. I talked to the vet the next day and they suggested I try some wet food for him. I went and got some and he seemed to really like it and was eating small amounts every few hours. He did sometimes seem nauseous after eating and did the chewing air/licking his lips thing as if he was getting some acid in the back of his throat. I gave him some Cerenia and it seemed to help at first but I could tell he was still nauseous the whole night. As soon as he tasted the Cerenia he immediately stopped eating, and instantly started drooling for about 5 minutes which I believe was from the bitter taste. This was Saturday night, and he has been very uninterested in food since, i’ve offered him about 5 different types of wet foods and he will smell it and then instantly do the nauseous chewing / licking motions after. I haven’t given him Cerenia since. I got him to eat a very small amount of unsalted butter the next day which was yesterday but he didn’t want anything else. Since he wasn’t eating he seemed to be feeling much better, even playing with his toy a bit, but obviously he needs to eat. I was really worried last night (Sunday night) and the only thing he seemed slightly interested in was his kibble. I gave him 4 pieces of kibble thinking that wouldn’t be enough to trigger his reflux reaction, but immediately he was sick and went through the painful vomiting motions twice within an hour. I then knew for sure to absolutely not give him any kibble at all, which is probably hard for him because he’s ate kibble his whole life so i’m sure he’s more used to it. Each time he throws up his mouth foams up before and his whole body seems like it’s trying really hard to vomit, accompanied by him cry meowing until he eventually gets it out. One time the force was so strong it knocked him over. Afterwards he seems to be nauseous for the rest of the night. Its incredibly hard to watch.

I brought him back to the vet again today and they injected him with Pepcid, gave him some fluids and took some urine for analysis. They also gave him some Mirataz to help stimulate his appetite. He was in good spirits once we got home but still wouldn’t eat. A few hours later he kept walking over to where his food usually is so I offered him all the different wet foods and he didn’t want any and would almost gag when he smelt them. Out of desperation I tried a churro which he was uninterested in yesterday, but today he was into it and ate about a tablespoon of it, but immediately again started to foam up and threw up again. I have an ultrasound booked for him this week as soon as they can get us in. I feel pretty defeated right now and just want to know if anyone has any advice or experienced something similar.

He hasn’t pooped since Saturday (it was normal looking and a decent amount). He’s been peeing regularly though. When he isn’t eating he is in good spirits and walking around jumping on stuff and doing his usual routine, purring and meowing and all that. Immediately after eating even if he didn’t throw up he would seem nauseous. After his puking episodes he will be tired and hide in his little igloo bed for a few hours and then will start to feel better again. Really not sure what to do and the vet seemed rather perplexed as well.

Sorry this was so long but I really appreciate anyone who reads it <3 Caspers my best friend and I want nothing more than to help him feel better.

Welcome to TCS! I'm sorry it's under such stressful conditions. The kitten designation is based on the number of posts you've made. You can't change it.

It's common for CKD kitties to experience nausea. I have a kitty who has chronic pancreatitis and he has chronic nausea. We give him ondansetron (Zofran) daily. It's a twice a day pill that's effective for 12 hours, but he's doing well with just a daily dose. However, for about a month, he got it twice daily. He also can have a daily dose of Pepcid, 1/2 tablet if needed for a faster acting pill if needed. If you can get the nausea under control, the eating may improve. We feed plain meat baby food when our kitties don't feel well. I like the BeechNut brand because it doesn't contain corn starch.

So now no matter how much or what he eats he's vomiting? I wonder if he has some sort of blockage leading to his stomach since it comes back out immediately :dunno:? I think an ultrasound is an excellent plan.

If your Vet truly thinks it's a case of acid reflux and you are having a hard time getting medicine into him, perhaps you can get the medicine compounded into another form. there are transdermal gels that you can rub into their ear, or even injections. We used to give our last kidney cat her prescription antacid via injections because she was impossible to pill. She never knew she ever got the shots because the needles were so tiny and we were to give them in her scruff. We would do it while she was in a purring frenzy getting some of her daily pets.

Thank you for the kind response!! I went to look for some plain meat baby food today but couldn’t find any at the stores in my town. I did get him to eat a bit of egg whites cooked in unsalted butter late last night and this morning, as well as a couple teaspoons of fancy feast in the afternoon. So glad he got some food in him, he does still seem really nauseous after eating though but at least he didn’t throw up. I just offered him some fancy feast again and it seems like he really wants to eat it and will graze it for a bit but then almost gags again.

I am also curious about a blockage, so the ultrasound will be helpful, although I assume if it were a blockage wouldn’t he immediately have been super sick? It seems like it’s gotten worse gradually over the past 2 weeks. Hmm.

I luckily got him to eat a bit of egg whites last night and this morning, as well as a couple tea spoons of fancy feast this afternoon and he didn’t throw up! Woo! He does still get a bit of acid reflux and nauseous after he eats, poor little guy. I’ll ask my vet about the Zofran! Currently giving him Pepcid injections every 24 hours as well as the Mirataz cream in his ear. So much better than pills.

Keeping any food down seems like a plus right now! And I'm glad to hear you are already doing the Pepcid injections. When is the ultrasound scheduled? Hoping it will give you some answers.

Early friday morning! Would’ve preferred to do it sooner but they have a specialist come in so he isn’t always on site. I will make an update on how it goes. So far Casper still hasn’t puked!! If he eats more than a teaspoon of wet food at once the acid seems to act up a bit so we’ve just been feeding him that amount more frequently. Still seems down and nauseous after eating but i’m glad his appetite is back.

UPDATE: Ultrasoune found a small mass on his liver which she said is most likely cancerous :’(

“Liver: The liver is normal in size with a diffusely heterogenous parenchyma. There is a poorly defined mass of mixed echogenicity within the right ventral liver that measures approximately the 2 cm in diameter. This mass appears to be causing dorsal displacement and compression of the gallbladder. There are additional similar poorly defined hypoechoic to heterogenous masses throughout the hepatic parenchyma. The common bile duct is not visualized. The biliary tract throughout the liver is unremarkable.”

So far he’s been eating a lot more regularly and has only thrown up once in the past week. We’ve been feeding him a teaspoon of food once every hour to 2 hours. We’re just going to give him the most love and keep him as comfortable as possible until his quality of life is no longer justifiable :(
