I would be extremely greatful if you would please complete this questionarre. I am a university student and I am doing an assignment about designing a pets website (much like the one you are visiting.) Here are the questions:

why don't you leave an email addy that we can use, so we can cut and paste the questions and answer them, then shoot them to you. That way you will have them for your own use, and not just on this forum.

I think you should set up a web page with a survey on it. That way you will be able to provide a link and get all the data at once.

I think that you'll get more responses that way.
