Kitten has had 2 BM's outside the litterbox...

I'm not sure where to post this, so I'll try here:

It's happened a couple of times now that Jasmine, 4 mo. old, has had a small bowel movement in the house, not even close to the litterbox. It's only the size of a grape, and only 1 piece. She always uses her litterbox otherwise, and I always am good about keeping it clean and scooped. It's not hard to get to it either. She seems healthy otherwise other than an occational sneeze that sprays some yellow mucus (I'm planning on asking the vet about that when I take her in for her rabies shot soon). I'll post a seperate post about that.

She's been dewormed and eats/drinks well but normal amounts.
Today when she pooed, my 3 1/2 yro saw it happen. The cat (according to my dh) was sitting in the steps, and ds says that she was just sitting there licking herself, then all of a sudden it came out. Dh says it was rather firm to the feel. It was once when I found it too. Otherwise she normally has BMs that are quite soft, but not diarrea.

Are there illnesses that can cause this to happen, or is it a behavioural problem?

She was a rescue that I got from a low funds shelter at 8 weeks, and she had worms, ear mites, fleas, an upper resiratory that I got her antibiotics for, and she was malnurished at the time. She's been treated for all her problems and she's gained a lot of weight + grown a whole lot since I got her. She looks real good at this point, well nurished. She's on Nutro Natural Choice kitten kebbles for food + sometimes their soft food, also some plain chicken/turkey now and then.

She is young, allow for accidents, give her more pans and just give her time.
