Maybe it is something in her diet causing loose stool or an allergy. This can be a mess in kitty litter, possibly your kitty is aware of this disasterous mixture and has found a way to not dirty him/herself. The carpet itself is probably stained of this smell which does mark bathroom areas for repition. After finding the cause of the diareah, a deep cleaning and a time release air freshner might do it.

Did the vet test for thyroid levels? I would be concerned about that in a 9 year old cat. My hyperthyroid cat occasionally poops on the carpet. It has repeatedly been in the same spot, so I know I haven't cleaned it sufficiently. You'll want to be sure to use an enzymatic cleaner designed for pet waste, like Petastic. Once it is clean, you can temporarily put a second litterbox there, and then slowly move the box somewhere else.
