Care document - any advice

Hey Everybody,
I'm working on typing up something for both the priest and I to sign that Claire is to be returned to me if anything happens to him. Our little rescue group doesn't have anything formal (although I think we should).

I think it can be pretty simple, with a statement that reflects the above, along with description of the cat, her approximate date of birth, date of adoption etc..., then both of sign it. I'm also thinking about putting something
in there about her not being taken to our country shelter under any circumstances. They have a very high kill rate. Just want to do whatever I can to protect Claire.

Does this sound reasonable. Any other suggestions?

Our rescue group has such a clause, saying that the cat can't be turned over to any other rescue group, individuals, or shelter without the explicit permission of our rescue group. We also reserve title to the cat until it has been vaccinated and spayed/neutered.

Personally, I would add that her vaccinations must be kept up to date, that she will be spayed (if she isn't already) and that he will not have her declawed. Part of the agreement my vet uses states that the cat must be kept indoors unless in a harness or enclosure.
