My Turkish Angora is acting VERY weird

Hello everyone, I'm kinda new to taking care of pets, my Turkish Angora has been through some serious times, he got his tongue split and was poisoned, he already fought with our dog 2-3 times and he also had some small diseases.

But I think this is the toughest one so far, lately my cat started to become more silent than usual, he will usually meow at almost everything, someone calling for his name, hearing plastic bags, seeing us with some kind of meat or when petting him. Now he only meows when he's scared or is stressed.

Our cat also started to just go straight to our backyard and lick the concrete/dirt, he will usually only go out of there when he's hungry or wants to do his needs, we tried to bring him to the kitchen, my bedroom and the living room, but he would start sprint towards the backyards without any doubt.

Today our vet told us he can't go for now, we need to wait till tomorrow, she told us he might have some parasite or might be sick, she suggested us to give him some a tablet dissolved in water and some serum. I brought him down to the kitchen and closed the door because I knew he would sprint towards the door, I tried to give him the medicine but immediately he spit it out with some foam, I was scared he had some kind of rabies but then he stopped to spit out foam. As usual he sprinted towards the door but he tried so hard to open it, he was scratching it, he was standing in two legs and started to meow a bit loud. I tried to give him the serum next, he just smelled it and proceeded to yet again try to open the door. Feeling bad I tried to calm him down, he did but I noticed his back was hotter than usual. After a few seconds I let him go and opened the door, I was surprised that he almost fell downstairs because of how fast he was going to our backyard. Seconds later I go up and see him resting there, he ignored me and proceeded to yet again lick the concrete/dirt. He didn't meow or anything, he was always giving me a blank stare without any movements like usual, no meows, no twitching tail, no licking himself, nothing, He isn't even doing his needs correctly, he sometimes just leaves a small poop there and then and a little pee, it's also unusual seeing how we sometimes confused his waste with our big dog.

TL;DR: My cat is pretty quiet and emotionless, his back is hotter than usual, he just licks the concrete/floor, he always goes to the backyard without any doubt, he isn't doing his needs properly, and doesn't want any kind of medicine.

Sorry if a few things are quite messy, I'm worried for him and I'm still learning english

Does he appear to be straining when urinating or defecating? If so, I would be concerned that this is a UTI, which is especially dangerous in male cats.

Unfortunately I haven't seen him urinate/defecate, even though we're keeping an eye on him, I think he only does that at night when we're sleeping.

However, he's been doing small amounts of poo/pee, his litter box is filled with small pieces of poop and big dots of pee, whenever he does out of it it's just a few small pieces of poo and a small puddle of pee
