Maybe try calling all the vets in your area and see if any have a recording with a phone # to call for emergencies.

It's a relief to hear that the kitten is finally delivered, and that mama kitty is doing okay. How wonderful that you have such helpful and well-trained neighbors! Hopefully the other kittens will be healthy and easily delivered. Please keep us updated.

So it was a deformity from beginning,  NOT because the kitten fastened?  If so, it increases the chance the deformed one was somewhat premature, and the body tried to get rid of it.  And the others will proceed to "cook" and will come out in their proper time.

Keep watch, and be prepared, but be also reasonably optimistic.

Also, its interesting.  Sheer despair, no help from anywhere, and suddely, there is both a helpful aunt and a breeder as neigbour, and even a helpsome vet!

Lesson?  There are almost always help to get, only to know from where.

Hello!
I do not believe the kitten that died in the process of being delivered, had a deformity. Of course, without seeing it, I cannot be 100% sure, but it sounds more like the kitten was breech, and probably too large to fit breech, and as such, died due to the cord being compressed with each contraction.
Sadly, kittens that die during a breech delivery will often present with swollen extremities, which CAN look like a birth defect.
Kittens that need to be manually extracted will sometimes have broken (and even dislocated!) necks, which may explain the kitten's neck length.
You were concerned that the kitten may have been a premature one. Was it hairless or sparsely haired? Did it lack claws or whiskers?
Kittens born preterm will often be hairless or nearly hairless. They may lack claws and whiskers, if born prematurely.
I ask if you are SURE the deceased kitten was premature, because if you are not sure, your cat will still be in active labor and should deliver the rest of the litter within 24 hours, not a couple of weeks.
Regardless, it is best to seek a vet's expertise, since you had to help deliver a dead kitten, and if the placenta was retained, if the mother is due, or if the kitten was indeed premature, it is best to see a vet ASAP, for the health of the mother and the rest of the litter.

Im essentially accepting moggielovers comments.   Its not sure the kitten was deformed for real, and perhaps not even premature (depends on its looks).   Still, it may have been started a full week before the others.  Not that common but not impossible.  So its what we hope for, but we really arent sure!

So, if your usual vet is an experienced obstetrician, its good to visit her to get a good check up.

You had alredy got advices from knowleable people, including this related vet.   Still, having a look see in home milieu, and having possibility to make ultra sound and thorough palpation by an experienced obstetrician, gives more safety....

Ouch. Real bad news.  So she was pts in the end anyway, even if it wasnt really medically necessary??   Really bad...  Im sorry and Im feeling with you.

May I ask, when did the other kittens come out?  Did the vets told when and why they died?

I do wonder a little, why they talked about an especial c-section when she was already through with birthing of the died kittens.  A common spaying should be enough... Priced as a spaying.

Something got sour.   Im sorry for what you had gone through...

You did the right thing by her, and I'm so sorry that you weren't able to keep her.

Breeding is a significant risk to mom. Be glad that you were able to save her life, and consider spaying your girls before their first heat in the future.

I'm so sorry, you have been through hell and it isn't fair that all this is happening. You did the right thing, although your heart is broken.  You will be blessed for your courage to do the right thing, it was a very unselfish act.  My prayers go out to you and that sweet little girl. I cry with you.
