We have a visit to the vet scheduled tomorrow morning. Tippy has always had a vomiting problem, but a bit more now. Also issues with her peeing and pooping outside of the litter box - like on the family room couch. She had a small lump on her belly last year which the dr said was a fatty issue, but I want to have it checked out. She's getting older now - not quite sure since she was a stray - but maybe 11 or so. Time to do senior blood work.

Tippy saw the dr this morning. He gave her a shot that is supposed to help the vomiting. Also doing senior feline bloodwork, so we'll have the results of that in a day or two. Small lump on her belly appears to be fat, will keep an eye on it and if it gets any bigger will send sample to the lab. For now we thought it best just to check the bloodwork first. Thorough scrubbed and cleaned litter boxes, but she already had one accident (before she saw the fresh litterboxes). I put her in the room with litterbox overnight and leave one outside for the other 2 cats. Twice a day I physically pick her up and put her in the litterbox. This has been an off and on issue for 10 years now. I think she's possibly just lazy. Will just keep on eye on her.

Has she ever had any bad experiences with litter boxes? My cat once had bladder crystals which obviously hurt when he pee'd. He associated the pain with the litter box so even after the problem was resolved he still wouldn't use it. I had to slowly re introduce him to it which wasn't easy. It took a while before he was fully comfortable using it again but he learned eventually.
Also have you tried different kitty litter? perhaps she doesn't like the one you are using for what ever reason.
Do all of your cats usually share the same litter boxes? If so it could also be a territory thing.
One last thought that you might try is reinforcing the good behavior when she does use the kitty litter box. Some sort of cat treat she loves or similar.

Hope everything goes well with the bloods
