Cat losing coordination after buprenorphine?

Long setup for question and long post:
I took my ~13 yo female cat to the vet Friday because she was spotting bloody urine all over the house. She has had FUS previously. I took her to the vet because the symptoms were unusually rapid onset and in frequency. Vet confirmed crystals in her urine, wanted to check for an infection. In clinic overnight, subQ fluids, cystocentis in the am. A single dose of buprenorphine (0.6 mg × 0.08 mL) injection was administered sometime Friday evening.
The problem: I brought Kiki home Sat AM and she was very uncoordinated and could just barely walk. Since she has been home, she has been increasingly lethargic and "drunk" when she tries to move. I can't tell if she's unable to walk because of pain or if she's uncoordinated from the medicine. I gave her one dose of oral buprenorphine
(0.6 mg/mL × 0.07 mL) last evening to test her pain level, but she still is not able to stand for more than a minute at a time. I was able to get her to drink some water this morning by syringing some into her mouth and then holding the water bowl right under her nose. Then she collapsed again. She cries when I try to move her from one blanket to another. Vet tech said she ate a little bit Sat AM. I need opinions on how to proceed. Husband thinks buprenorphine knocked her down, and I should just wait and see what happens. How can I feed her to give her a chance? Could the buprenorphine be causing the lethargy and not being able to move?

.6 seems like a very high dose it could be that. I don't think my cats have ever had one dose over .2 or .3. Better to ask the vet ASAP but this does seem like a dose to wipe out any pain!!

When I multiply the dose out -- it's 0.047 mg injected, then 0.042 mg orally about 24 hrs later. I've been able to get her to drink a little more, but she is struggling to stand. Thanks for the advice

Ok that sounds better. She may not be able to stand due to the pain. Did you ask the vet what could be happening?

You would have to check with your vet as to the type of buprenorphine they used in the injection.  There are two long-acting formulations of buprenorphine, one that lasts 24-48 hours and the other that lasts 72 hours.  Obviously if they used one of those, you probably would not be giving additional at home so soon.

Also, the concentration that our vet uses is 0.3 mg./ml, not 0.6 mg./ml.  I do know that the dose is supposed to be tailored not only to the weight of the cat but also to the pain level, and if it is too high for the pain level and/or weight, the cat can be somewhat drunk, but normally they are not, they are just sleepy.  The recommended dose of buprenorphine is 0.01 mg. to 0.04 mg. per kilogram (2.2 pounds).

I too would suggest calling the vet, and if she's having trouble walking, I wouldn't give any more of the buprenorphine until you talk to the vet.  As for the bloody urine and crystals in the urine, she should be on a high ANIMAL (not vegetable) protein wet food diet with moderate fat and LOW carbohydrates (under 10%), which will naturally make her urine slightly acid which will prevent struvite crystals forming.

Kiki crossed the rainbow bridge. She didn't respond well to the emergency treatments. There was some kind of inflammation in her paw
also. She was facing 4-5 days ICU hospitalization for a doubtful prognosis. I couldn't put her through that, so I let her go a little while ago

Kiki was far sicker than I imagined. She presented severely dehydrated, in sepsis, and her blood work indicated her liver and kidneys were already shutting down. This all happened in such a short time

I am so sorry to hear that you lost your sweet Kiki. It is so hard when our kitties get so ill, and it seems to happen so rapidly. They can seem fine and then they are not. It is also hard when there is never truly a diagnosis. It did seem as though Kiki was far sicker than anyone thought. Just know that you did all you could for her and she loved you for it. My heart goes out to you during this difficult loss. I have been there, and I would never wish this pain on anyone. It is truly heartbreaking.

RIP Sweet Kiki!
You were a special kitty and will be so very missed!

Oh no I'm so sorry. It's so good that you did take her to the ER you helped her so much with that and addressing her distress so quickly. Rest In Peace Kiki....

Thank you all for your support. She left her paw prints on my heart, and took a piece of me with her. She will be dearly missed

I'm so sorry for your loss and know you did the right thing for her.  The buprenorphine certainly would have reduced her pain and made her comfortable in the last days of her life.  When you feel up to it, you can post a tribute to her with photos in our Crossing The Bridge forum which is dedicated to all of our furbabies who have gone on ahead.  We all know how heartbreaking it is to say goodbye to our much loved cats.
