Sore under tail

My cat has a red, moist wound, with no hair, under his tail, just above the anus. I got advice last night to put an e-collar on him and put Neosporin on the sore 2-3 times a day and watch for any improvement. Today, he had brown crusty bits around his anus and a little bit at the edge of the wound, looks like a small amount of leakage, or liquid that came out with his stool and dried there (everything looks normal in the litter box). When I took the collar off for a few minutes tonight, he tried to wash the area almost immediately, so I'm thinking it's some combination of anal leakage and over grooming the already-irritated area, but I'm not sure. Anyone have ideas or suggestions? He does hack like he's trying to cough up a hairball fairly often, but rarely gets anything out, and he sometimes makes a hiccuping sound, usually when he's purring. Could this all be related to some sort of digestive issue? I started adding some pumpkin to his food today, but it's too soon to see if it helps. He is on a wet food diet, but a couple weeks ago I added a small amount of dry food before I go to bed in hopes that the cats won't wake me up for breakfast so early in the morning. He's been on dry food before, but not this brand, however it's only about 1/8 cup so it seems unlikely to wreak so much havoc.

Any help or suggestions would be appreciated!

Has he been to the vet? He sounds like he has multiple issues going on and really needs to go. I would be concerned about a ruptured anal gland.
