After looking it up it appears to be a "cuterebra" infestation. I'd never heard of it before but that hole looks exactly like what I found.
The cat really needs to be seen by a vet asap. Basically, it's the breathing hole of very small worms who will later hatch into a type of fly. You can not care for this yourself as it isn't that simple to remove and clean out.
I'm assuming the cat in question was found as a stray? That would make it much more likely.

It does look extremely like the fly sivyaleah mentioned, I saw a graphic vid on youtube, please don't look if you have a weak stomach.
I wouldn't rule it out if your cat is mainly an indoor cat, it could have come in via vegetation brought into the house. The fly is primarily found in the northeast US and southeast Canada.
I live in Kenosha Wisconsin, so midwest. It is really odd, when we saw it if you can see the black dot on the photo there was a flea in it we think and pulled it out.
I do not think it could be any type of infestation either since there is no hole going down any further.
