I have named him Archie. He is emaciated & full of fleas & worms. The vet thinks he's only around 2 years old.
Breaks my heart. He has been given fluids & the vet gave me an iron supplement to give him since he is anemic but he may he a blood transfusion.
I've rescued many, many cats but have never seen one in such poor condition.
I have also syringe feeding him because he will not eat.
Any advice to help Archie?

Oh, poor, poor dear.  You are an angel to help him.

I have no advice - just keep doing what you're doing.  Send prayers Archie's way.

Have you ever heard of bone broth?  It really helps sick kitties and allows them nutrients and fluids.  It does take 24 hours to make, but is fairly simple in a crock pot.  I will post a video below.

Offer tuna juice too.  It is often enticing and sometimes they will get little bits of the tuna too.  Can the vet give you some AD prescription cat food?  It's for sick kitties and very palatable.  I would only be feeding wet right now.

I had syringe feed a very sick kitty last year.  I would blend wet food with chicken broth or tuna juice in my magic bullet.  I then would give syringes of bone broth too.

My thoughts are with you that this dear sweet one will pull through.

We had done the same thing four years ago - an all white cat with big blue eyes ended up on my doorstep. Fully grown and only four pounds. Could feel every bone in her body the vet said if we hadn't taken her in she would have died within a day or two. Four years later she is now 13 pounds and thriving, her name is Arthemis. We basically just hand fed her canned food - literally just put Fancy Feast on our finger and shoved it into her mouth until she had the strength to feed herself. Keep doing what you're doing and he will be better in no time! Bless you for taking the sweet darling in x

This was Artie the first day:

And this is her now (sorry they are screenshots from Instagram; I just cleared my photos off my phone!):

There definitely is hope!!!

Prayers for you and Archie. Did the vet give him a de-wormer? Archie may be too sick for Frontline flea medication, but you could comb the fleas off him with a flea comb and that would help. It's hard for a cat with fleas and parasites to get nourishment. Keep syringe feeding him both water or broth as well as pate style canned food mixed with water, every few hours if you can. Be careful not to force the food down too quickly so it doesn't get into his lungs. Get as much guidance from your vet as you can. Tell Archie you will love and care for him if he will fight for his life. I brought a sick feral practically back from the dead this way. Please let us know how this little angel is doing. Thanks for your kindness.

Despite my very best efforts, Archie earned his angel wings at 2:15am October 1st.
Archie had FIP, a deadly disease which has no cure.
R.I.P. Archie, I loved you, even if only for a short while

Archie was sent to you so he'd have a loving home the last day of his life. He died with dignity, rather than as a nameless cat who was thrown away, unwanted, and sent to the dump for burial. Five years is the average life span for cats on the streets. It is a very hard life, and many wind up as sick as Archie, and suffer greatly before they die. TCS family, please make a donation to a feral cat rescue, in memory of Archie and so many others who died on the streets. Bless you for taking Archie in. He is free of his sick body now. May he rest in peace.
