The horrible smelly brown stuff sounds like anal gland secretions. Your cat may need her glands emptied. The vet can do this for you if you've never done it before.

Sometimes the anal glands get full of goo. Most of the time the glands are emptied normally as the the cat poops. In some cats, that might not happen for various reasons so the glands get full and may start leaking brown stinky goo. So you have to empty the glands yourself or have the vet or a groomer do it. There are online videos of how to express a cat's anal glands yourself if you don't want to pay the vet to do it. It can be a messy job so don't wear your good clothes and do it in an easily cleanable place like the bathroom and use old towels or paper towels. You may need a second set of hands to hold the cat.

Some cats get impacted glands that may need vet treatment.

If you can get your cat to lose the extra weight, she will be able to reach her back end and clean herself up. 22 lbs is a very unhealthy weight for a cat.  There are many threads here on TCS on how to help a cat to safely lose weight.  You may find this particular thread helpful: http://www.thecatsite.com/t/245236/mickey-my-27-lbs-foster-and-his-journey-back-to-a-healthy-weight

The problem could be coming from the anus or the vagina, it is impossible to know without an examination, but whichever it is it does need a vet. It could be something simple or more serious, and needs medical advice. Please try and get her to a vet. Her weight could be a factor - without cleaning herself she is more likely to get infections. So to get her to lose weight could help prevent this kind of thing in future. Can you give us an idea of her feeding schedule - maybe we can help you work out a good diet to help her, though you will have to be strong and resist feeding her too much - I had a terrible problem making my diabetic cat lose weight. But it is worth it for the advantages it gives the cat.

Here's one video: You can find many others online if you jsut search for them. Basically you use your fingers to firmly squeeze on both sides of the anus. You can gently push the skin in a bit and massage the area while squeezeing to remove any goo that may be inside the glands. Don't squeeze too hard. Here are pictures: http://www.wikihow.com/Express-a-Cat's-Anal-Glands

I can empty my cat's glands myself but for many other people, a second person is needed to hold the cat still.

A second vet opinion is also best. Some vets don't have much experience with cats so they miss a lot of stuff. And if no anal gland secretion comes out, your cat may have an entirely separate problem and that needs to be examined by a vet. Look into low cost vet clinics in your area.

Quite honestly, I would not recommend an amateur doing this procedure at home. It would be very easy to rupture something, and unless you are holding the cat absolutely correctly, the cat will probably not let you get near his butt! You need to understand the anatomy of the cat to do it, it is not like getting a tick off, or even taking a temperature (and that is not recommended either unless you really know what you are doing. Please do everything you can to get the cat to a vet - after all, you don't even know that this is the problem. She might have an infection as a result of her spay
