Diarrhea, vomitting, and change in behavior-when do we need to go to the vet?

My male, senior kitty of 10 years old fell a 12 foot drop outside about a month ago. We went to vet, the vet said that he seemed fine (no broken bones), said his teeth looked good, but did notice an ear infection, and gave us some ear drops.

He finished the ear drop treatment about two weeks ago.

The last few days, I've noticed his poop has been looser. He has some firmer stool, and some that's really loose (for lack of a better image, like frosting). He hasn't been burying his poop in the litter box, and has been pooping in the front corner versus the back. He's been getting poop on the floor near his litter box, and stuck to his fur as well. 

I've been cleaning his litter box 2-3x a day now, and he still is getting poop on himself, and smeared everywhere as well.

His litter box is a large rubbermaid container inside a huge tote with a hole cut out in the front (to dog proof).

It's been reoccurring for 4 days or so now, and not getting any better. 

He hasn't been acting weird, until this morning, when I found out from my boyfriend he's been vomiting brown liquid once a morning for the past 4-5 days. This is before eating breakfast, and he get's dinner around 9pm-11pm depending on my work hours.

Haven't changed his diet, he currently is on Sheba Pate chicken, fish, and beef, and free fed Iam's senior diet. 

I've been wanting to switch him to Blue Buffalo, but didn't want to upset his already upset tummy. 

This morning, he didn't want to eat breakfast. Which is NOT like him, and he had a lot of poop in his fur and he didn't groom himself after I trimmed/washed the area. He jumped up to his top cat shelf, and is snoozing away.

He won't eat cooked chicken or tuna, or any form of cat food that's not a pate or dry food (I tried various brands and types of food when his sister had jaw cancer, he wouldn't touch any of it). 

What could be causing reoccurring loose stool and daily vomiting?

When should we take him to the vet? Does he need to go in ASAP? He's been acting normal otherwise, today is the first day he didn't want breakfast and is acting a bit more lethargic than normal.

Yes, I think you shall go to your vet. Not necessarily the ER vet, but as soon you can arrange it.

An overall tip; goat milk may be worth to try.  Esp if you can get raw.   But visit the vet anyways.

Thank you for the response. I made an appointment tomorrow for 930am, I unfortunately can't go in today because of work. Ill be sure to keep everyone updated though and let you know how his vet appointment went. They didn't say too, but should I bring in a stool sample?
