Post spay large hernia, how dangerous?

One of the feral kittens I TNR'd had a small hernia at incision site post op. It is now 3x the original size. She has become a bit domesticated but doubt would be amenable to vet visit.

Does she need surgery?

Not to the vet, but to the office receptionist who of course said I'd have to have her examined. Problem is she is more feral than not; also from past experience, though I have brought many many cats to them due to my TNR program, I do not get a break. One cat needed most of his teeth removed at a cost of $1000, then was asked for further surgery to examine a bad eye rather than examine it while under anesthesia for teeth.

I just feel they would recommend surgery. I live in a sparsely populated area with not many options.
