Hyperactivity after Simbadol (Buprenorphine injection) - 17 y/o hurt cat supposed to be NOT mobile..

My cat, Lance, is a 17 year old Siamese mix. I was petting him earlier tonight when I realized he was clearly in pain around his hindquarters.. I've never seen him show pain like he did (distressed meow, growling, fake biting at me.. he normally purrs constantly being around me, regardless of where I touch/pet him..). So I brought him to an emergency vet clinic..

The vet said that he most likely has some soft tissue damage in the area. She prescribed an antibiotic (just in case there's a bite hiding under his fur) and pain medicine for home. She also gave him an injection of Simbadol. It was recommended to limit his movement as much as possible - especially running, jumping, and stairs..

We got home about a two hours ago. He is acting completely drugged, but in a hyper way.. He won't let me get near him. He won't let his best friend, Maki, get near him. He's mostly hiding in some of his 'safe'/hiding places. But if anyone makes a move towards him he bolts and ends up in another hiding place.. He's sort of stumbling around and generally seems incredibly out of it; ie. running in circles, acting very twitchy and suspicious, etc.

Since returning I did manage to get ~1-2 oz of wet food into him. AFAIK I know he has not had much water. I can't get him to come near me for treats.I am really concerned as he is now way more active than he's been in years, which is quite literally the opposite of vet's orders.. I also need to medicate him with the antibiotic and thyroid medication, but that's rather impossible considering I can't get near him. I'm sure that I could physically grab him if I really tried, but I do not want to encourage more movement or stress him out more..

The cats are already under an incredible amount of stress due to life situations. He also (for the first time) showed a heart murmur today, which the vet says is due to either thyroid problems and/or stress..

I've tried just sitting across the room from him, which he tolerated. But he still ran if I made a move towards him. I'm currently in my bedroom (upstairs), because I thought maybe having a break would help. I just went downstairs to check and it has not.

Please, any ideas about how to get him to calm down? What to do about meds? Should I try to have him be around me or just give him his space in the hope that he'll calm down?

I'm just looking for any thoughts or ideas. Thanks to anyone who read this and those that may have some thoughts to offer.

Some cats react this way to this particular pain medication. My cat recently had the same problem. We kept her closed in the bathroom with food, water, and everything that she needed. I would suggest putting him in a smaller space if you get a chance to catch him.

Some cats react this way to this particular pain medication. My cat recently had the same problem. We kept her closed in the bathroom with food, water, and everything that she needed. I would suggest putting him in a smaller space if you get a chance to catch him.
Sorry to hear about your cat - I hope she's feeling better!

Unfortunately, for various reasons, there is really nowhere to enclose Lance (nowhere that he won't freak out anyways). My plan was to keep him mostly in my room, with food and water available, and carry him downstairs at certain intervals to use the litter box and such. I'm scared to try this right now, as I don't want him just to run and cause more issues..

Its definitely a scary reaction to drugs when they're supposed to be resting. I had the worse time keeping my cat still. Even in the bathroom she was bouncing off walls. She was recovering from a surgery to remove some problem toes, not supposed to be running around since she was likely to break the adhesive they used to close the wounds.

I don't know what you can do if there is no where to put him. I suppose try leaving him alone and hope he calms down.

Yeah, it's been terrifying for me.. I'm truly sorry you had to go through a similar situation.. How's your kitty doing now?

Update on Lance:

Last night - after giving him some time alone - I was able to give his hyperthyroidism meds (using transdermal). I couldn't get him to try the antibiotic and I didn't want to force him, so I skipped it. I was able to grab him and carry him up to my room, where he calmed down mostly, although he didn't sleep.. (And of course early nighttime he's usually always asleep - gotta conserve that energy for early morning so he can wake me up.. 
)

I brought him downstairs a bit ago to medicate him and allow him access to litter box and more food/water. He immediately started racing around again and then hid in his safest spot. I did get both meds into him, but he was very unhappy about it. Right now I'm giving him a bit of time to hopefully calm down and then I'll bring him back to my room.

He is eating (wet food, bit of treats) and drinking (fresh water), but it seems to be less than usual. I don't know if this is a side effect of the drug and/or stress and/or jumpiness post-medication.. He seems to eat/drink more when I'm not  with him, which is something that's never happened before.. Should I be concerned already (he was eating/drinking normally before we saw the vet. We've been back home for ~13 hours).

I think as long as he is eating it is okay.

My cat is doing very well. I found that she calmed down more as time went on so hopefully the same happens with Lance. I hope he's able to calm down some in the next few days.

Which pain medication are you giving at home?

I'm glad that your cat is feeling all better!

Lance is back up in my room now, laying next to me and purring away. It appears he did have some more wet food and water - and just scarfed down some treats in here (Feline Greenies dental).

I haven't started the home pain meds yet, as the vet said to wait ~24 hours after the Simbadol, unless he seems like he's in extreme pain again. It will be Gabapentin 25mg in oral suspension every 8-12 hours as needed. I'm really hoping that he handles it better than the Simbadol..

That was last night - we were snuggling and watching a movie together. Or I should say that I was watching a movie, while he was busy relaxing and purring so loudly that I had to keep upping the volume.
