Older Cat Change in Behavior

On March 11 of this year, I adopted a 10 1/2 year old cat named Simba. He had been at the humane society where I adopted him from for over 5 years, so I decided he needed a forever home. I have two other cats that came from the same humane society (Spock and Buffy), though they never crossed paths with Simba. 

Simba has several issues. First, he has ear polyps in both ears, most on the outer portion (though one vet I saw did say he has a few internally too). They don't seem to bother him too much, so both vets I've seen have said it's best to leave them alone, unless they start bleeding (which they haven't). Simba also came to me with an issue of throwing up... a lot...several times per day, in fact. However, it got progressively worse (to about 12-15 times per day), so I took him to the first vet. The first vet jumped to the conclusion that Simba had FIP, since he was positive for the coronavirus. I then took Simba to another vet, at the first vet's direction. The first vet wanted Simba to have a biopsy, but the second vet said she didn't want to poke him unless she really thought he needed to be; so, instead, she gave him an ultrasound. Both vets said all of his blood work came back normal and the second vet said she really didn't see anything abnormal on the ultrasound; she said she could see a few small lymph nodes, but nothing that worried her. The second vet diagnosed Simba with IBD, so he's now on Hill's Prescription Diet i/d formula (dry and wet). He seems to be doing well with it and has only thrown up 3 times since the visit with the second vet (which was on April 15, 2016). Simba is also on 1.8ml of amoxicillin (twice per day) and takes 1/2 of a tablet of Pepcid AC per day. Simba can be a picky eater, but he hasn't had a loss in appetite.

Within the past 2 days, Simba's behavior has changed a bit. I refer to him as my "grumpy old man," because he sometimes fusses more than my other cats, but recently he's gone beyond being just grumpy. He's started hiding under my bed and will hiss/growl at my other cats and myself if we get near him; last night when I tried to give him his amoxicillin for the night, he became extremely agitated and I had a hard time settling him down (I was able to finally give him his dose, however). This morning, he didn't put up his normal fight when I gave him his morning dose, which should be a good thing, but still worries me. I tried to see if he had a fever, but I don't have a thermometer I could use on him.

I don't want to call the vet if I'm completely overthinking this change in behavior, but I wanted to get some thoughts on if this is "normal" or not.  Like I said, his appetite hasn't changed and his bowel movements are pretty normal from what I can tell. I'm just worried about my grumpy old man. Since I've only had him about a month and a half, I'm still not quite used to his "normal" behavior; according to the people at the humane society, he was never considered a lap cat and would have his grumpy moments. Any thoughts would be appreciated! (sorry for the lengthy post)

I am bothered by the hiding under the bed. Good that his eating and litter box habits have not changed. But I don't like cats hiding. We want them to be confident, up in the world, owning the territory, tail straight up.

It could be medical especially given his past. Personally I would mention this to my vet.

How did you introduce all the cats? The usual way? I wonder if something happened when you weren't around a couple of days ago.

Does Simba play? I always think of this as either medical or confidence driven. Can you play with him and then give him a treat (even if it is his wet food). Does he go up in a cat tree? Have you seen any indications that the other cats have intimidated Simba? How are their interactions?

Since you have had him for a month and a half it seems like maybe something has changed. It could be nothing but I would call the vet and also try to build his confidence. I don't know how he was acting around the house and how confident he is but I would try to pay attention to him, talk to him, pet him if possible, play with him if possible etc. All stuff to say I love you and this is your home.

I hope this kind of helps, please feel free to ask anything and no apology need for your post length. The more info  the more helpful it is to find a solution.

I agree, you are an amazing human being for taking in Simba and for giving him so much love and care. I agree with CalicoR that it's concerning that Simba is hiding and hissing/growling. Is Simba taking amoxicillin on a full stomach? After or with his meal? Amoxicillin on an empty stomach can cause a lot of discomfort and pain. And was the Pepcid AC prescribed for his vomiting? If he has IBD, Pepcid AC isn't going to treat that. Pepcid is really to treat excess acid and heartburn.

I don't like Pepcid because I've seen humans get dependent on it. They start taking it and then get trapped in a cycle of heartburn and relief with Pepcid. Once you stop taking it, the stomach goes right back to producing excess acid, then they have to start the meds again, then the body continues to overcompensate by creating more and more acid. It's really not a solution to any problem except to create dependency on the drug which is EXACTLY what all drug companies want from their users. I also just don't like the fact that he's on those two meds at the same time. If he has IBD, he should not be on Pepcid AC!

You're obviously doing everything you can (and then some!) to find the right treatment for Simba. There is one other place you can ask. It's a free app that you can download to your smartphone called Kuddly. I do NOT have any affiliations with them, but I suggest it to all my pet owning friends because there are some great cat specific vets from all over the country on there who will offer advice and ideas via email/skype/phone for a nominal fee (under $10). It might be worth a shot to do a filtered search and shoot a message to one of them.
