I saw my cat with a worm coming out his anus, he has diarrhea, and some discomfort.

Cats and worms seem to go together. The recommended thing would be to take the cat to a vet, along with a stool sample if possible...or bring the worm in. The Vet can ID the worm and give the proper worming medicine. There are many different types of worms and not all are killed by the same medicine, so identifying the worm is suggested. Over the counter worming products usually work well, but again, you need to know the type of worms that need to be treated.

The vet will be able to determine what kind of worm it is and prescribe the proper treatment. Follow the vet's exact directions on how much of the medicine to give and how often. One dose will not work. it usually takes a couple doses spread over anywhere from a couple of days to a week or so to fully break the worm life cycle.

OTC medicines are too broad spectrum and may not work on the specific type of worm your cat has.

If it was a squirmy little worm that was about the size of a piece of long grain rice it's a tapeworm segment.

This requires specific treatment, either from a vet or you can buy what is called Tape Worm Tabs, like at Petco pet supply stores.

These work but are the only over the counter ones that do work.

Tapeworms come from chewing fleas off a cats body.
