Prednisolone & Behavior Changes

My 14 year old cat Chessy was diagnosed with kidney disease in August.  Ever since, he has vomited a few times a week, usually clear/white foam, which is a symptom of the kidney disease.  He also has frequent diarrhea.  I've tried canned pumpkin and slippery elm bark, and sometimes the pumpkin helps a little.  We've been doing subq fluids every other day and Pepcid AC twice a day.  We also did a little Calcitriol twice a week but he's off that as of Friday, because he seemed to vomit the morning after more often than not.  His kidney values have stayed pretty stable since August (he just had bloodwork done again Friday).  His BUN is about 60 and his creatinine is about 3.0.  Phosphorus and calcium normal.  

He was also diagnosed with diabetes in February, but we tapered his insulin to his actual blood glucose levels by testing at home, and he has not needed any insulin since March, so he's not considered diabetic anymore.

I took him in for new bloodwork on Friday because last week he vomited more than usual, and instead of clear/white foam, it was bile, hairballs, and then undigested food.  Although his kidney values were about the same, the new thing in his bloodwork was one of his liver values was much higher than normal.  His ALT was 199, and it should be under 100.  The vet said she can't do a definite diagnosis without a bunch of other tests, which we just went through with our other cat Mia.  The vet's guess was that he might have lymphoma, based on his other tests and symptoms.  She said we could start giving him a small dose of Prednisolone, 2.5mg daily.  He hasn't vomited since.  (If he does have lymphoma, I wouldn't want to put him through chemotherapy since he's 14 and has kidney disease also, so I just want him to feel better.)

His mood and behavior were normal until about an hour after his dose of Prednisolone tonight.  He started growling and making strange loud noises for no reason.  We were in the same room but not near him and not paying attention to him.  Then he went upstairs and has been hiding under the bed.  He came down once, walked over to my husband, who pet him lightly, and then he growled and went back upstairs under the bed.  I've checked on him a few times and he looks ok.  He might be sleeping now.  I'm really upset because I've never seen him act like this.  I read that Prednisolone can sometimes cause aggression and behavior changes.  I texted my vet (she gets a lot of texts from me) about tapering his dose, and she said to try the same dose again tomorrow to make sure it's a pattern.  I don't really agree with that...I think it's obviously due to the steroid.  


My other cat Mia is currently getting 7.5mg Prednisolone twice daily for IBD, so 6 times more than him, and she weighs less, and she doesn't have any negative side effects, except maybe some lethargy.

I don't know what the best thing is...I need to follow what my vet says, but I'm afraid he will only get worse.

Has anyone experienced this before?

Chessy woke me around midnight and was back to his usual purring loving self. Maybe his body just needs to get used to the steroids?

Sorry I have no experience with the medication, I am glad things are getting back to normal with Chessy.

I know a lot of people use prednisolone, so I'm hoping you'll get more replies with people sharing their experiences. I haven't used it myself, but my friend does for her cat and he didn't have any personality changes. Hopefully it's an adjustment to the new meds and he will settle down after a couple of days.

My kitty is on Prenisolone and he has had no behavior changes.  Each kitty reacts to medication differently though.  Hopefully this was a one off thing and your kitty will be okay with the medication..

There's a bit to discuss concerning kidney disease. I currently have a miracle cat who was diagnosed with it at age 16. He turned 23 ( yes ,you read that right) on December 12, 2015. I have another cat who used to create kidney stones but is in perfect health now. Right now your question centers on the reaction to prednisone. If your cat has demonstrated such a negative reaction to the drug, I would indeed question further administration and amounts. Vets notoriously do not provide certain information in reference to this particular drug use. Did you know that the majority of cats when placed on this drug for a short period will have a tendency to develop diabetes in about 6 to 8 months ? Everyone reading this post ,ask your vets from now on . It has happened to me several times before I finally figured it out and confronted a top radiation oncologist. However sometimes there is no other option but to use the drug. You do the best you can. Ok. Next. What else are you doing for your cat to reduce inflammation? I read you tried slippery elm and pumpkin. Slippery elm would be an outstanding option. Do you have trouble administering it? It can be made into a gel or placed in a smaller capsule if just sprinkling the powder in food does not work. It should be given at least 2 X per day. I have given small amounts of it 4x a day followed by home made chicken broth or food when necessary. Pumpkin is excellent for constipation in IBD patients. It also builds platelets so if tolerated, it would be a great addition to your cat's diet. However, you stated your cat had experienced diarrhea . I would suggest High quality probiotics, Greek yogurt, if tolerated, and 100% psyllium with NO OTHER ingredient ( Whole Foods carries one. It's organic and under their 360 label) . That will help the stool issue. Back to your kidney problem. Are you familiar with the online company Only Natural Pet ? I have been using their Kidney Support for many years with several of my cats including the 23 year old. Additionally I use Vetri Science 's Renal Essentials . Home made chicken broth if made correctly can do wonders. I don't know how far you are willing to go for your cat. For me, I leave no stone unturned but that's my DNA. Hope this helps.

I must agree with zen warrior on all of the suggestions. Sometimes cats do have to be on medications, but from my experience it appears that prednisone is the "go to - cure all" medical treatment, And the side effects can be horrible (especially on the liver and kidneys). That being said, countering the internal side effects with natural remedies has proven successful in so many cases. The behavior changes normally are a result of the way that the medication makes the cat feel. Only Natural Pet has some great options and I agree that probiotics are a fantastic option to help with IBD. There is also a product called Oxy-Cat which helps with IBD and cellular function. We had to give our cat a few months of pred last year and she had no apparent reaction because we gave the highest quality wet food with natural supplements to counter the possible side effects. Hope this helps.

Zen Warrior, that is amazing that your cat just turned 23!

Yes I did know about the diabetes risk.  I had no trouble administering the slippery elm, and Chessy actually liked it, but it did absolutely nothing to help his vomiting and diarrhea.  Pumpkin also didn't help either of those.  Chessy doesn't have IBD, he has kidney disease and possible lymphoma.  My other cat Mia has IBD (separate thread called Vibes for Mia Please) and she was at the brink of death, and prednisolone was the only thing that helped turn her around.  I tried a little probiotics with her, but right now she needs to put on as much weight as possible.

Both my cats are eating grain-free Soulistic canned food, but I will also check out Only Natural Pet, Renal Essentials, and Oxy-Cat.  I haven't heard of those, I'm interested.

Artie has been on the steroid since May or so. It has really helped him. I have not found any personality changes at all. He suffers from IBD. It has really cut down on the inflammation. In fact, if anything, I can honestly say that my kitty came back...he was so sick before taking it. 

I am aware of the diabetes association with it. The specialist checks his blood every 3-6 months. 

I was told that the steroid does not have the same side effects on cats as it does on humans. They can tolerate it pretty well.

It's Only Natural Pet Kidney Support, and Vetri Science Renal Essentials as supplements. Pumpkin is for constipation and support for increasing blood platelets. I had a dog diagnosed with lymphoma at age 6 . Got 4 years. She died cancer free but from a genetic problem/ mitral valve prolapse. I currently have a cat diagnosed with nasal lymphoma. She's been in full remission for 2 years and doing spectacular. The 23 year old cat in addition to having kidney failure was diagnosed with eye cancer at age 6. I was told by age 10 the ophthalmologist would have to remove his eyes to prevent the cancer from getting to his brain. Nope. Never happened.. Still has both eyes fully functioning. I jam him with supplements and an arginine diet which clearly arrested the cancer. Using supplements for your cat's kidneys is the way I would go. The 23 yr old and 2 other cats are on both supplements I listed early in this response. Research it and see if they work. You must judge accordingly
