BACKGROUND: fixed

former stray cat in 2005 that was near death and save our strays spend $700 to get him in good physical heath.

now about 12 years old,

From the title of my post you may think this is normal. Ongoing for 24 months, However the past 2 weeks the meows have become louder and more deep vocal.  And increasing. Almost always he is with a stuff animal or stuffed toy. Sometimes he will meow into the air. We adopted a shelter female cat 8 months ago that is full of energy. He lightly chases her but she  is not afraid of him.

This morning, had the notion, could there be an underlying medical issue?  Most surely i will bring him to the vet next week.

Yes, there could be an underlying medical condition.  Get him to the vet for a senior blood panel and also check his thyroid and for diabetes.  I, too, would be concerned by this behavior.
