My 4.5 month old kitten Luna is having some potty problems and the vet can't figure out why.

Some history. I have had Luna and her brother Maroo for about 2 months and I feel like their poops have always been very smelly, soft but formed, and a really really dark green color. I didn't realize this was not normal until recently. My vet says they should be brown and not have a very strong smell. Also, 2x about a month ago I noticed there was pee outside the litter box. Didn't know which cat was the culprit and it stopped so I didn't pay it much attention. Then, Luna recently (about a week ago) passed a stretchy string that I pulled out from her rear. After Googling I realized this can be extremely dangerous. This is what prompted our recent series of vet visits.

I took her in and let them know about the string and poop issues. They said to keep an eye on her and start her on fortiflora to see if that will help. Both babies were given their last round of FVRCP that day and we went home. Doc said exam was normal so no immediate concerns. The next day I saw Luna in the litter box and lift her butt in the air, kind of like downward dog pose, and pee ends up outside of the litter box. She will twitch a little bit then get out. Basically straining/discomfit while trying to urinate.

I took her back to the vet right away and they did a fecal and urine analysis and came up with nothing. Don't see signs of swelling or blockage. She is on a pain medication (Buprenorphine) and antibiotic (metronidazole)"just in case" and to see if it changes any of her symptoms. I do not like this approach because I know there are risks with medication so I would rather know the cause before giving her anything. However, I am not the vet and they seem to think this is the next best step. The next step will probably be switching her food to see if that helps.

Has anyone else had this experience? It is very troubling to me because she is very young and that the vet can't pinpoint a cause.

Thanks in advance!

Did your vet take x-rays to check if there was anything else obstructing the bowels?  Is Luna constipated? Buprenorphine can cause constipation and sometimes what looks like straining to urinate is actually straining to poop? I'm confused, metronidiazole is sometimes given for diarrhea; do you know for what reason your vet is prescribing it for Luna?

What are you feeding them?  Have they been checked for parasites using a fecal float as well as a PCR test for diarrhea?  Did the vet do a culture and a urine specimen to check for crystals or urinary tract infection?

The vet has not done X-rays. They said it probably wouldn't show if there was more string in there. I wanted to do X-rays but just went by what the vet says. Depending on what happens after these antibiotics I will probably insist we do them. The metronidazole was prescribed for diarrhea/soft stools. The straining is happening during urination and was happening before meds.

Sorry I accidentally sent the reply too soon. I feed them ziwipeak canned food. I was also giving them half of the airdried version but have stopped that. The doctor said I might need to try another food but I don't feel great about any others (Low carb/fat to protein ration, low phosphorus). The fecal and urine tests came back negative for parasites/bacteria infections/stones/crystals. They have not done a culture yet. I'm wondering if an ultrasound/blood test should be done.

I would ask for a urine culture, especially if straining while urinating.  I would also advocate for an x-ray and blood work.  The x-ray may not show anything, but it might.  The bloodwork is the same. It might not show anything, but again it might.

Generally, soft, smelly poop is due to food intolerance issues.  I'm not sure where you live, but you might check out the food and nutrition forum for food suggestions.

Those were my thoughts as well. I don't like the idea of just giving her antibiotics to "see if it helps" when we don't know the root cause. I will most likely try a new food to see if that helps but the Ziwipeak seems like it has such good ingredients :(
