need advice again .IBD Possible Kidney issues

In the past week I've made a few posts about my Lucy and her IBD issues. For the past 2 weeks she's had a bad flare up of some kind and has been vomiting alot no matter the food. She lost alot of weight and has been snubbing food alot. On Thursday 10.15.15 she vomited a very large amount basically emptying her belly. Since then she's eaten only a few spoonfuls of food. On Saturday I took her to the vet and he didn't seem to care about the IBD issues and after looking at her chart from her last visit April 2014 he said her kidney levels were high. Creatine 2.0
Urea nitrogen 56
Bun 28. Other than the urea the others are still in normal range?
I told him she's having all the same issues she had originally in 2014 and I'm more worried about her not eating.
My main issue is that her belly makes insane grumbling noises. She ate nothing on Saturday and wouldn't eat anything today until I gave her so low sodium deli turkey. She loves this and I only give this to try and stimulate her I to eating. But as soon as she swallows her stomach makes the noises. If the noises go on long enough she will vomit again and her already inflamed belly will be worse. I have pepcid and Seb but can't get the seb I to her .

What do I do about the stomach noises? I have about 13 different brands of canned cat food ranging from.chicken,lamb,venison,Brushtail,kangaroo proteins and she wont touch any of them. Also as soon as the Venison would enter her belly (2weeks ago) she would vomit

I'm at a loss. The vet did his blood and urine tests and I'm waiting foe the results. Being that she was so dehydrated at the time of the tests wouldn't her kidney levels be off anyway?

What do I do now ?

I posted in the yahoo forums but everyone just says see an I internist/specialist but with $7000 already spent on 2 cats over the last 3 years I just can't afford one especially here in NYC

All I can sayis "yes", the values would be off if she was dehydrated.

As to what is causing her stomach issues, I wish I had answers for you.  I know you have been dealing with this for quite some time, and it's awful for both you AND especially Lucy, poor girl.  I believe, if I remember correctly from your other posts, you have tried just about everything already, including novel proteins. 

Has anyone mentioned EPI?  I know it's a longshot as this usually presents with diarrhea, but sometimes ir presents with vomiting. 

I'm guessing she's on anti-emetics?

Hi Joey.I'm sorry your girl is still struggling.We do find Internist's most of them far superior to any vet.Yes that costs money but I know with my Maxwell if i added up all the botched vet visits and tests(telling me nothing) it would have paid for the internist 2-3 times over.Is she on something for nauseau .Cerenia or Ondanestron? As for SEB what form are you trying to give her? If she is not on something for nausea i would ask for cerenia now.If she is hard to pill it comes in injection form as well.has anyone mentioned adding prednisolone?This may be an option now as the inflammation can be huge.Maxwell started at 10mg daily & was then tapered down within a week or so.Also a pill .The growling sounds alot like my late tabby who I could hear from the couch.She had pancreatitis.Only diagnosed with a blood draw Spec Fpl.You do not have to have a specialist make that blood draw.She was given metronidazole which you can talk to your vet about as well & she did recover at 15 yrs old.Other name for it is flagyl.If nausea isn't addressed as per a med daily she may never eat.Cerenia helps mostly with vomiting.Something is going on in her gut.Does she vomit right after eating or you syringing or is it hours later.It it digested food.non digested food.Foamey stuff? i would be asking your vet for cerenia.I would be asking your vet for a spec fpl test.I f pancreatitis it is very painful.Alot of kitties have to be on pain meds like buprenorphine.Your vet will not know if she has pancreatitis without the spec Fpl.he/she may assume that's about it. yes with dehydration you will see a rise in kidney values.creatinine will rise with dehydration>if you have her Blood work in front of you you can post them to the CKD groupa t yahoo & they will read it & tell you if anything is related to not eating,vomiting etc or something else that you should be asking your vet about.Again very high volume group & You do need to post large new member need advise Please Help.There are over 14,000 members there that can read BW like the back of their hand.most dealing with inappetance,vomiting  & all that you have experienced. If you do not have her Bw & hopefully they did a urinalysis as well you re privy to all test results of your babe from your vet.Some will email them to you direct.The Ckd group will walk you through the numbers and tell you what is of their concern.Super people!!!But you are right food is life.!!!! But pls join and at least get another perspective on kidney values.But nausea has to addressed.I you used Cerenia & it did not work the next thing is Ondandestron.A human drug requiring a script from your vet & purchased at a human pharmacy.the CKD group will know all dosings etc.And your vet should know but don't be surprised if He/She does not.

The Seb is made by "Now" it is capsule form and I've removed the powder from the capsule and followed the directions from the site. I ordered a different brand that is powder form so I will try that. As an update I was able to get Lucy to eat some baby food that I saw on tonyas site but that's it . I guess its a start but as soon as she swallowed her belly started making the gurgling noises

Hi Joey.Like Stephen & I both agree nausea has to be addressed.My Maxwell had bf every day with pumpkin & babyfood butternut squash with his miralax.If she ate probably gerbers stage 2 turkey that little jar is over 100 calories.But this food has to stay down.There needs to be a diagnosis made.An anti nausea med tried like Cerenia.the noises you hear I explained what is possibly happening in her gut.They are not normal as you already know.the seb are you adding to meat & syringing it to her or what?The brand os SEB shouldn't matter but I can't say for sure.Tanyas' site is a huge wealth of info.but honestly you need to take her back to the vet.if you are not happy with that vet pls find another.She is not well.it can be a minor thing or amajor thing but a diagnosis has to be made.i understand being at witts end.This is why we pay the big bucks.For answers.If she is nauseous all the food in the world won't entice her.If she has small cell prednisolone has to be introduced to help bring down inflammation.Inflammation causes pain & a slow emptying gut.Called Ileus. Vomiting occurs etc..pain makes cat's not eat.IBD is treated with pred as well.I belong to the IBD group & most members there, Their kitties will be on pred the rest of their lives.It's just the way  it is.They try lowering the dose to say 5mg per day but I haven't seen any lower.They usually start at around 10mg & then work downward.Has your vet talked to you about that? It is dirt cheap.Please take some of our suggestions, write them down or print them off & as soon as you can afford it pls take your baby in.She is not well.You know that.I wish there was a magic answer.But unfortunately there could be one thing or many things going on with her.If you haven't tried pred then ask your vet that you want to try that next & cerenia for vomiting &  nausea.ok? My kitty's IBD was controlled for several years with 2 1/2 mgs a day.  He recently started loosing weight with no other symptoms so had the ultrasound and lab work done again with nothing unusual showing up.  We increased his dose to 5 mg and he has regained the weight.  I have seen kitties here with IBD controlled on 2 1/2 mgs every other day.

Hi Denice thank you for your input. That is great to know.What diet are you using for your kitty and have you through the yrs had to change it?Does your kitty have the sounds coming from the tummy like the member's cat?So obviously with your kitty losing weight inflammation increased requiring you to have to up his pred.So every kitty inside their gut or bowel all depends on  just how much inflammation is in there & also if it is just IBD or has changed to carcinoma.Pred is used highly for both.I Appreciate you sharing as you can obviously see on this forum as well as others there are many kitties in the world dealing with is.Hugs C.

I spoke to the vet just now and he said her kidney values we slightly elevated and her liver enzymes were slightly high and he suggested an ultrasound which would have been no problem but they don't have anything available until November 13. He suggest I take her to the aspca to have the abdominal ultra sound done. I called the aspca who also has no appt available until November or I'd have to come in as an emergency patient the charge is 450-550 plus $85 .
I called 7 other vets and no one has anything available for atleast 10-15 days and all are above $400 plus visits "specialist charges"

Wouldn't the liver enzymes be bmhigh because she's not eating and the kidney values being slightly elevated due to the dehydration

Hi Joey. First off I would never have an US done by any vet.25% is machine & 75% is brains.He/she should be sending you to a Board certified Radiologist..Yes it cost money.But I have seen personally many vets who went out & bought a machine to accout for their high pricing (to pay off all these machines) to make many misdiagnosis'

Here is blog to read about why to not have a vet do one.If you can't afford it pls ask your vet that you want to start your baby on prenisolone.

Maxwell went off food completely & was vomiting.I syringe fed him for three months & yes pred was introduced starting at 10mg daily & slowly tapering down.You must have seen Denice's post re pred & IBD kitties.I would ask for a referral to a Board Certified radiologist only.Start the pred now.And it is not instant.We are not vets nor specialists as per your kitties numbers.There are groups out there like I told you that read bw everyday & will tell you if it is related or something else.We can make all presumptions.But they may likely be untrue.A true Board certified Radiologist will look over her whole body,kidneys,gut,pancreas,liver,etc.taking measurements etc.But as for the pred it can be started today & is cheap.

If your pet ever needs an ultrasound exam, it's crucial to ensure that it's done properly. An inadequate ultrasound can be worse than none at all, since it often leads to misdiagnosis, which can be very dangerous for your pet.The use of ultrasound in veterinary medicine has become very common but there are no regulations in place to ensure that your pet’s ultrasound is performed competently.

Ultrasound isn't magic; it takes training, skill, and experience for the operator to accurately locate and examine the various abdominal organs and tissues and to correctly interpret any abnormalities. Without adequate mastery, it can be very easy to misinterpret an ultrasound image—to mistake gas bubbles in the colon for bladder stones, to miss subtly enlarged abdominal lymph nodes, to overlook a localized intestinal thickening. Some organs can be very tricky to locate if the operator lacks proficiency; it is common for inexperienced individuals to miss the adrenal glands or pancreas, for example.

If your pet requires an ultrasound exam, it falls to you to ensure that the person performing the procedure has adequate training to do so effectively, since there are no standards currently in place in the veterinary profession. Anyone can buy an ultrasound machine and start using it on people’s pets, and the results of mistakes can be tragic.

The best way to protect your pet is to have the ultrasound exam done by a board-certified veterinary radiologist if possible . Radiologists undergo a lengthy residency program, during which they receive rigorous training in the performance of various types of ultrasound procedures. To become board-certified, they must pass an arduous certification exam. If there is a veterinary radiologist in your area, this is the best person to perform your pet’s ultrasound procedure. To locate a radiologist, you can go to the veterinary radiology website and search for a board-certified radiologist in your city or state.

If you can’t find a veterinary radiologist in your area, a veterinary internal medicine specialist is another good choice. Veterinary internists also receive ultrasound training during their residency programs, although their ultrasound training is not as extensive as that of a radiologist. Internists are also skilled at the interpretation of abdominal ultrasound results, since the abdominal organs are one of the areas in which they specialize. An ultrasound exam of the heart (echocardiogram), for example for a pet with a heart murmur, should be performed by a board-certified veterinary cardiologist; you can find one in your area here.

One method to avoid for your pet is the “remote ultrasound”. In this situation, instead of having a specialist perform a full ultrasound exam, someone takes pictures using an ultrasound machine, and sends them to a radiologist for interpretation.Unfortunately many mistakes can occur with this method. If an abnormality is not recognized by the untrained person performing the exam, or the images are of poor quality, the radiologist cannot make an accurate assessment of the animal. This technique should be a last resort, for geographic areas where there is no radiologist or internist available.Owners should be given the choice of seeing a radiologist or internist and made aware that this option exists.
