I have a 12 year old siamese mix cat. She hasnt been feeling well for about 2 weeks. I dont live at home, but my mom noticed she had rapidly lost weight one day and wasnt eating her food (dry food).  That day my parents gave her tuna water and bought some wet food which she ate a bit. After a couple of days of wrangling with various foods, I took her to the vet.

Vet did a CBC and urinalysis. and sent my home with an appetite stimulant cyproheptadine and some canned a/d

at first she would eat the a/d but has completely lost interest in it

results came back- no thyroid problems, BUN and creatinine levels normal, no glucose in the blood,no signs of liver problems. however he mentioned a new screening for kidney disease SDMA , normal values 0-14 she was at a 20. but its apparently an early detector of kidney disease?

The main issue he said was her high WBC count, so he suggested IV fluids, abx or oral abx. I couldnt afford the IV, so we opted for Orbax.

At the time he wasnt able to retrieve any urine, so I later brought a sample 4-5 days later- no WBC in urine, or bacteria.

Shes been on the orbax for about 10 days now, and I havent seen much of an improvement. She doesnt eat much, barely has 2 oz of wet food a day, doesnt eat any of her dry food and drinks water on/off. Shes been urinating regularly, and at the begining was missing her litter box but now is consistent. Bowel movement every 4 days or so because she barely eats.

I dont know what else to do, the Vet suggested I see an internal medicine specialist, but I can't afford that.

please help!

We can't really help with diagnosing your cat, even your vet is having trouble.  Imaging (x-ray or ultrasound isn't cheap but may be worth a try, especially the ultrasound which is better at looking at soft tissue.)

You may want to consider syringe feeding, there are good links on youtube on how to do it, search on "syringe feeding cats".

Also, there is a way of giving your cat sub Q fluids, your vet could teach you how and sell you the supplies.  Many people do this at home for diabetic cats, and i've done it and with a little instruction and 2 people, you could do it too.  Thinking this through, your vet would have to give them the first time and would likely charge for that but often vets have something called a "tech charge" which is a lower cost charge when they have a vet technician do the work instead of the vet.  I'd ask about that.

I understand, I have been reading alot of posts on here and was just looking for some advice. What are some typical infections for cats, a higher wbc indicates infection but it wasnt a urine infection or dental infection....
does anyone know anything about SDMA values, she was found to have 20 which is an elevated value.

I have tried syringe feeding, she wiggles out of the towel
And escapes most of the time. I can get her to eat some wellness can, but its a verrry small amount.

Hi there my Kitty is going through kidney problems too.  Someone sent me this web site  for anything kidney related.  http://www.felinecrf.org/index.htm   Maybe you can find more info there.  Also my vet checked kitty's blood pressure and that's was up too.  So now she is on meds for her blood pressure which is helping with some of the kidney stuff. Mine isn't losing weight but she is gaining it fast, I did change her food to an all natural diet per the vet.  I give her and OTC food that is low cal and she is eating like a pig.  Hope the web site helps you.

I'm wondering if her kidney values are really that significant to be causing this problem? The vet told me SDMA is for early screening and shouldn't be the culprit because her creatinine and BUN values were normal.

I not sure all these tests and numbers you could call a different vet to get their take on it.

Did the vet check her teeth and mouth for ulcers?  What about an x-ray to rule out any blockages?

Is she pooping?  Sounds like something going on in her gastrointestinal tract.  Maybe a blockage??  I don't know.  I would ask for an x-ray.

I would definitely get an x-ray to rule out anything internal.  I wish you the best.
