Bald Patch - dry skin, thinning fur?

Last night when I was snuggling with the beast I noticed that the fur around his neck, where his collar rests, had thinned considerably and the skin seemed very dry. Almost as if he had over groomed and chewed it off but it's in a spot that he absolutely could not reach. When I run my fingers through his fur normally I am able to part things and comb things over and see his skin underneath but this was considerably thinned out and a very clearly a bald spot. 

His collar is very loose, I doubt very much that it was too tight and rubber the fur away. He is indoor only so it is not a flea collar, it's just a fabric collar with his ID tag. 

He had no sores, no wounds, no rashes, no bugs or fleas. Not even any redness from over scratching and clearing the fur off. It was normal, non-irritated skin without fur where fur should have been.

He is eating, drinking and using the box very normally so I see no other symptoms presenting that would be alarming, and while I am not one to shy away from taking him to the vet (when he needs to go I do not hesitate and move quickly), I am a little hesitant at this point to go because if it's just a symptom of being old I'd like to avoid the stress of car rides and being poked and prodded. 

Has anyone else had something similar with their kitty friend and found topical, over the counter or at home remedies that were helpful? Would love your feedback and suggestions. 

I have included a little health history to cover the basic questions:

Gizmo is a 13 year old male, fixed, indoor only short haired cat who is in reasonably good health.

He has had previous issues with constipation that was diagnosed as Pre-Megacolon which we are treating successfully with daily Lactulose and Grizzly Salmon oil for lubrication and omega-3's. 

Doc noticed that he had lost a bit of weight at his last checkup and attributed it to the Lactulose working so well that he wasn't able to absorb all the calories he needed to maintain his body weight before things moved out of his system. So in addition the the Lactulose being mixed into his food we have also been adding Goats Milk with great success - he loves it and he hasn't lost any further weight. Goat's Milk is high in healthy proteins and one of the most digestible milk products on the planet, even the most sensitive tummies can digest it without a problem. I get it frozen at PetFood Express.

He is on an all wet diet (Wellness Brand) and has a water fountain available 24/7 for fresh water.
