Metacam and clavaseptin issues

Hi everyone, my 12 year old Tortie Nala went to the vets today as she has been acting very strange and hasn't urinated since Sunday. Got xrays, urine test, and blood work done, all showed an infection. Our vet put her on 5kg of metacam for 2 days (gave her a dose at the vet) and clavaseptin (gave her a dose at the vet) she seemed fine, when we got home she started acting like she was drunk, and went into the bathroom went behind the toilet and sat there for hours and hours before I finally picked her up and moved her. She had her head pressed against the toilet the whole time. I'm very concerned as this is not like Nala at all. Any information is helpful, I am going to call the vet but I thought I would ask for opinions as well, any experiences, ect.

I should also add that she has two sores from laying down so much this week. One on her tummy and one on her leg

Hi nalacat12 and welcome to the forum !

Did the Vet remove any urine from her bladder ?

If she has lost no urine whatsoever since Sunday...and, if she were my cat, I would be considering this a medical emergency and I would have her seen by a competent feline Veterinarian ASAP.

It is the height of incompetence to prescribe/give a non-steroidal anti-inflammatory - NSAID - to a cat with urinary issues, most especially a cat with no/restricted urinary output........Metacam is one of these drugs, is only processed through the kidneys, is nephrotoxic (it destroys kidney cells), and so, it circulates and re-circulates continuing to damage the kidneys.

A urine blockage, itself, can severely damage a cat's kidneys, and this is only compounding the problem.

I do not understand, either, the choice of clavaseptin.....that is an antibacterial, usually used for skin infections.

Did the Vet do a culture of the urine....did s/he know exactly what kind of bacteria was present?

This would be an enormous concern for me and I would rush her to competent care immediately.

I would expect her bladder to be emptied...carefully...some incompetents will try to "express" a bladder (use physical pressure to force out the urine) without determining whether/not there is a physical blockage in/of the urinary tract ...this can be done safely if there is no physical blockage (like stones/crystals)....if force is applied to a blocked cat, the bladder can be ruptured. The urine can also be safely removed by other means.

I  would also want her to be put on IV drip so as to speed the removal of the Metacam.

Secondarily to all this, I would be looking for a competent feline Veterinarian....perhaps start by looking for a cat-only (NOT just "cat-friendly") clinic.

I wouldn't be surprised if Nala will have some residual kidney damage from this.

Hi white shadow,

We went to two vets one was just a local vet who prescribed her the metacam, and did the xrays, after review on the vet ww found out he is under review for misorderly conduct, so we did not give her the metacam straight away, we went to another vet (feline specialist) today and here's the story for that below

We got to the vets and she started by doing a physical check up, took her temperature than proceeded to tell us what her plan of action is which was first to review the xrays, get a blood sample, than to get a urine sample, and lastly an ultrasound.

Xrays, and blood work came back clean. Urine test came back "fuzzy and dark" and she said when the urine looks like that it means either crystals or possible mass. They reviewed the urine and found a small crystal in it. The xrays showed no blockages and blood work showed nothing wrong with kidneys ect. Only UTI. She said to give nala the Metacam for today and tomorrow to help with any inflammation and the clavaseptin for 4 days for infection, she said if that doesn't help we will need to do an ultrasound to figure out what's going on. She also told us that on review of the xrays that there was no sign of any urine in her bladder and she thinks because nala has a UTI that she has been going pee in unusual spots in the house which is apparently not uncommon for cats with UTIs.

I gave the vet a call and she thinks that nala is very stressed because she is not used to being man handled and she had to get needles ect. Done today, she said if she is not better tomorrow than to call her and we will go from there. I will definitely bring up the points you have provided with the vet and see what her opinion is, thank you for the reply
