Very Low Neutrophils (White Blood Count) Need Advice Please

FIV is transmitted thru biting, not scratching, if that helps. And, honestly, I've never heard of needing to do a bone marrow aspirate to check for it. There are TWO different tests, but neither is that. They are both done with blood smears and if they come back negative, that is usually good, as far as I've heard. WAs he vaccinated for this? Not sure that simply waiting a month for another CBC is really going to tell you anything, since Petey's numbers seem to fluctuate like the tide.

Is your Vet a feline only Vet? If not, do you have access to one? I'm thinking that may be your best bet...try to find someone who specializes in felines. They are better versed in all things cats and may have some idea of what might be wrong. (still wonder WHY it couldn't be that hemobartonellosis you mentioned way above

Thank you @mrsgreenjeens! That is a huge relief that scratching doesn’t transmit it. I couldn’t stand it if both my nuggets were sick. We have done multiple blood smears and all have come back negative for FeLV and FIV but the vet seems to think it’s one or the other. She said sometimes he doesn’t show up in the blood but only shows up in the bone marrow.

I don’t know any vets in my area that specialize in feline only. I will try to google it but the few I’ve asked have said no, there are no specialties in their practice. I’m not sure I want to spend another thousand dollars and put Petey through a painful procedure when we might not get any answers. I thought monitoring the WBC count was the way I might go for now.

He has also had irregular GI issues with 2 big flare ups in the past 18 months (not this incident although he has had diarrhea) where his GI wall was mildly thickened and slightly corrugated. They also bounced around the term pancreatitis as a possibility. I’ve spent $10,000 in the past 18 months and I still have no answers. I would spend another $10,000 if I could properly diagnose and treat him but I am losing faith. I googled all day (at work!) and I’m more confused than ever.

I got all his CBCs sent to me and these are the numbers.

Neutrophils:

GI incident last February 2/20: 2435

Anemic incident 8/27: 1220

8/28: 930

9/2: 1358

9/10: 767

10/1: 870

And, of course, if you CAN find one, then you could get ALL Petey's records from your current Vet and take them, so he shouldn't have to have the same test redone (I wouldn't think), saving you lots of money, and possible lots of extra trips in to see them. They might even be able to review everything first without even seeing Petey (just guessing here, but it's possible). It might even be possible, if they are not close, to get them to look at his records and offer their opinion long distance...again, just guessing.

I had decided to wait another 3 weeks and do another CBC and if neutrophils are still low, do the bone marrow aspirate. I sent all Petey’s records to two other vets. One of them got back to me saying his lymphocytes are very very high which could be what is pushing his white blood cell count down. From google searching: “Significantly elevated lymphocyte counts can be indicative of leukemia as well as other illnesses, including hemobartonella, IBD, hyperthyroidism, fever of unknown origin and cholangiohepatitis.”

This brings me back to IBD. I feel like this is partially to blame but I don’t know. I have a message into the vet who is not so great at getting back to me. Trying to decide if I should have the scope done to test for IBD and/or blood tests for toxo and FIP but I really doubt it’s either of those based on his current symptoms: basically none. He has full appetite, full playfulness, mild diahrrea mixed with some solid. I wouldn’t suspect any problems based on his behavior.

Also, I am sick and this worries me. I’ve seen conflicting feedback about humans passing colds/flu to cats. I have a flaming throat and possible fever. I need to ask the vet if he should be on antibios but … not returning my calls.

I have a couple of suggestions possibly that might help you & your kitt.First if you have had and ultrasound which I'm presuming you have in the small intestine are 5 layers to the wall.Depending on where they are seeing the inflammation will depend on whether endoscopy would be accurate or not.With my boy they felt the little jaws that grab the samples would not be able to penetrate deeply enough and the result would come back negative.The only trued diagnosis is made with and exploratory.As for FIV belong to a very small group of FIV kitties & owners.Joel the owner a super Guy.A lot of knowledge there.As for your Bloodwork,inflammation and all other symptomology I cannot say enough about the pancreatic group at yahoo.They simply are brilliant.They do not just deal with pancreatitis but any inflammation,blood work & have been around the bend with almost every ailment a kitty can have all over the world.When joining this particular group pls put in big letters new member kitty has very low neutrophils (Help) something like that.the FIV group will walk you through their process of joining.both being very easy to join.

Thanks @MomofMaxwell! I requested access to the pancreatic yahoo group. I made a spreadsheet of his numbers. White blood cell count seems pretty normal. They neutrophil is very low and the lymphocyte is very high.

He has no symptoms of anything right now, my vet is being completely non-responsive, and i sit at work and try to diagnose him all day. You would think after spending over $10k the vet would try a little harder.

First column of numbers was his first incident and overnight hospital stay. 2nd colum (2/20) was one year later, same symptoms (diarrhea, lack of appetite) and coincided with adopting his brother Charlie who he now loves but he was very stressed at the time. Then the last 5 columns is this illness which did not start with diarrhea but strange behavior, heavy fast breathing, hallucinations, he was severely anemic. I was just watching the RBC and hematocrit levels and was thrilled that they went from low to normal, not realizing the neutrophils were a problem.

I can't stop googling and researching. I don't know why I didn't think FeLV was very likely, I guess because Petey has been tested multiple times and the tests have come back negative (blood tests). The reason the vet has suggested the bone marrow aspirate is to test for FeLV/FIV in the bone marrow. I was planning to wait another 3 weeks then get another CBC but after doing more research on FeLV I am scared to death. If that is what Pete has it's not jsut him, it's his brother Charlie who is constantly grooming Petey (even in the butt area) shares a litterbox and often eats and drinks from the same dish.

PLEASE if anyone has any knowledge of FeLV especially in a multi cat household please let me know.

From that same website I referenced before (Cornell), they say there are two tests for FeLV, and neither of them is a bone marrow test. Here's their article: http://www.vet.cornell.edu/fhc/health_information/brochure_felv.cfm.

You know I believe YOU can consult with Cornell Feline Health Center. There may be a fee, not sure, but it seems to me that I remember someone discussing that once in a thread. Yes! Coincidentally, I just got off the phone from an Internal Medicine Consult. On his CBC's there's a phone number for vets to use to consult. I talked to her for 30 minutes, she thought I was a vet.

Basically she did not believe me when I told her Petey received no medications and his anemia went from 14% HCT to 38% in 4 weeks with no intervention other than his initial blood transfusion bringing him up to 16%. A week later he was regenerating - no meds, no steroid shots. She didn't believe me. Petey has been sick 3 times in 15 months, each a mystery.

She said some cats just have a low neutrophil count and the only way we know if this is the case for Mr. Pete is if we got a CBC when he was healthy (which we don't have). She said the 2 times prior when he was sick his neutrophil could have been elevated BECAUSE he was sick and his normal is just around 500 to 1800. Petey hasn't fallen below 768 and he came up 100 between 9/10 and 9/30. She said if there was an issue like FeLV, his count would be going lower, not even up by 100 (which I thought was negligible).

She said unless he has latent FeLV, it would show up on the ELISA test. If it is latent, then he's not shedding and not contagious.She said she would continue to monitor his CBC, she would not do a bone marrow aspiration unless his neutrophils fall below 500.

As an aside - I think the IFA is done on the bone marrow only. Not sure though.

Now i belong to an FIV group very small group owned by a fellow Joel.Super man.very compassionate people.If you want to talk with people who are dealing with FIV kitties & comparing notes this would be the place.tested at the shelter you said for fiv & felv.Depends if they did a snap test or what.A snap test is not very reliable which is what most shelters do..You can read that online as well.I understand this grasping but if you had the assumption your boy was fine as per felv & FIV which you have you wouldn't checkout that route.Unless retested which results are emailed.They don't take long.There is absolutely no excuse to be waiting days.Here is the link to Joel's FIV group

Now i belong to an FIV group very small group owned by a fellow Joel.Super man.very compassionate people.If you want to talk with people who are dealing with FIV kitties & comparing notes this would be the place.tested at the shelter you said for fiv & felv.Depends if they did a snap test or what.A snap test is not very reliable which is what most shelters do..You can read that online as well.I understand this grasping but if you had the assumption your boy was fine as per felv & FIV which you have you wouldn't checkout that route.Unless retested which results are emailed.They don't take long.There is absolutely no excuse to be waiting days.Here is the link to Joel's FIV group

Petey was tested both at the shelter and again a few weeks ago when he got sick. They used ELISA which is highly reliable as well as RealPCR. Negative all around.

Petey has had 5 CBC's in 5 weeks. I'm waiting on the bone marrow b/c it is very invasive and it doesn't seem like FeLV is really a viable issue. I don't know, but I trust this doctor I just talked to from the lab. I would spare no time or expense for my Petey, I have spent every penny I have and then some - $10,000 in 15 months is more than most people would do I would guess.

You google sign up for a yahoo account.Not difficult at all.Free I didn't say felv but was mentioned in my post because the link I sent you mentions it &.FIV.I realize the money you have spent and i also have a filing cabinet full of vet bills.$5500.00 for 99 days of life for my Trixie.i would sell my house for my kitts.i understand.You may trust this person at the lab but you have to realize you have not gotten any answers and spent alot.And major stress having multiple blood draws for your boy.Although a necessity.Although the money means nothing like me.You need answers.Sometimes you will get the right answer from sharing your problems with someone.No one here has given you a direction to go.To say they have dealt with exactly the same problems and this is what they did.Specialized groups like Lymphoma,crf,etc are just that.FIV can rear its head & the symptoms i don't know of nor do I think you know either.Yes we are grasping.To make a yahoo account Honey & I am no computer geek is preventing you from a wealth of groups that can help you now & in your future.It's free & google walks you through the steps.I think doing that and taking your mind off the major worries may help you clam down just a little.Your boy feels everything you are feeling.They are truly so sensitive.

All these groups are sharing.Caring & learning.Some groups dealing with specific feline illnesses.They are truly a god send to all of feline lovers. C.I am grateful to be a part of them for as long as I have.So grateful & have learnt so much.

Petey is only 3 and I find this so hard to grasp. She also admitted the other symptoms I described have her confused.

Petey has had 3 serious bouts of sickness in the past year and a half. The other 2 we did tons of testing, overnights, big 3k bills with no results. NO transfusions but no appetite, diarrhea, peeing outside box, etc. They thought maybe IBD?

After TONS of googling I did see a couple of sites saying low blood counts could be due to stress. I believe Petey is very sensitive to stress and my household has a large dog who barks and over reacts in a pretty stressful manner to anyone just walking down the street. Petey runs and hides every time. I have actually tried to find a better home for the dog who I love but he needs someone home all day and I believe he is making Petey sick.

Does anyone have any experience or info they could share with any of what I’ve said. I am still in a state of shock that they think Petey has cancer, he is so young and he is my favorite little boo, I truly cannot imagine my home without him in it. Please help?

My cat has the same exact thing happening to her. She has been scratching/excessively grooming, has a massive scab on her neck, and has very low RBC, WBC, and platelets. The vet said she needs a transfusion and suspects bone marrow cancer.

Did you figure out what was going on with Petey??

The person you are asking this question of hasn't been on this site since 2017 so I doubt she will respond. I see you have started up your own thread, which is the best thing to get advice on this matter.
