My 14-year old cat was recently diagnosed with triaditis.  He recently had a flare-up and was given a one dose injection of metacam for the pain as well as an injection of buprenorphine.  He had explosive diarrhea all weekend and became very agitated and barely slept for 3 days.  On the fourth day, we took him back to our vet and he had comprehensive blood work done. This is when the official diagnosis was given as the vet explained that his liver functions were not in the normal range (indicative of cholangeohepatitis).  She wants us to begin prednisolone immediately, which I have yet to administer.  I am concerned about the metacam injection that he had four days ago.  I heard that one should wait at least one week before giving steroids to a cat that was prescribed metacam.  Our vet assures us that because the metacam was a one-time dose (supposedly it was only in his system for 24 hours), beginning prednisone will not cause harm.  Can anyone relate to what I am experiencing?  Is it ok to begin the steroid as my cat was only given one injection of metacam versus being on it for a period of time?  Any suggestions and advice would be greatly appreciated.  Thank you.

I am so sorry your cat has been diagnosed with Triaditis which is actually three conditions occurring simultaneously: pancreatitis (inflammation of the pancreas), cholongiohepatitis (inflammation of the liver), and inflammatory bowel disease or IBD. Did the vet do a biopsy - exploratory surgery to diagnose? A GI Panel? I had foster cat years ago that was diagnosed with this condition and unfortunately - a rescue with a health history - this cat was given Metacam injections in the past for his condition and I believe this was what caused the inflamed liver and kidney issues. He sadly also had stomach lymphoma and did not live long :shame: - had multiple health issues, poor thing. Anyway, Since your cat only received the Metacam injection after the diagnosis, then I would not blame the Metacam on the liver issue since your cat only has had one injection. I would be against giving a steroid to a cat that had a Metacam injection, though, and would wait a full week's time to begin Steroidal treatment. I would ask the vet about adding in an antacid as well. I really don't know too much about this condition other than it is important to manage symptoms to keep kitty comfortable by diet, pain management (I would not use Metacam, personally - I would go with Buprenex alone), anti-inflammatories, antacids and even adding sub-q fluids. It is so important to have a vet you can fully rely on and trust as well to help manage these symptoms. Your vet does sound very thorough and what we suggest here in this forum either by experience or other is to be taken with a grain of salt as we are not vets.
