Male spraying in litter box

Hi, all!  New the site, looking to see if anyone has any ideas for an existing problem we have with our cat Tom-Tom.

We adopted Tom-Tom from the Washington Animal Rescue League in Washington, DC last June (nearly a year ago now).  He's an awesome cat, really.  He's got a fantastic, unique personality and my fiancee and I adore him.  He has a continuing problem, though: Spraying in his litter box regularly.

I've looked up dozens of sites that all tell me similar things like, "change the litter type," "maybe he's anxious," "maybe he doesn't like his litter box," etc.  However, after trying a number of recommendations based on these possible problems (got Feliway, changed litters, changed litter boxes, changed locations, added water next to the litter box, giving him more attention, using non-enzymatic sprays to clean the litter box), nothing seems to be working.  He's been to the vet a few times now (once came back with a UTI diagnosis and was diagnosed with Feline Lower Urinary Tract disease).  When he was on antibiotics for his UTI, he didn't spray at all.  It seemed like when he finished, however, he went right back to spraying.  He was also given anti-inflammatories once, as well, but that was of no help.  He's been put on a strict, special diet of Royal Canin Moderate SO wet food and dry food.  He doesn't spray all the time, but he still sprays the vast majority of the time.  Tom is a big fella (about 11 pounds), so we feed him two cans of wet food a day (once in the morning and once in the evening).  His water intake is fine and he finishes his wet food with vigor.  He is seemingly totally fine other than when he urinates.  He does not strain or cry when he urinates, he only shakes his behind, sprays, and hops out.

My latest stop-gap measure has been to get a storage container that has high enough sides so that when he does spray, it stays in the litter box.  But I'm at a loss at this point and don't know where to go from here.  If anyone has any thoughts or suggestions, I would love to hear them!  Thank you so much!!

I don't know how to explain why the problem stopped with the antibiotic but can tell you about my cat Wesley. When he was about 12, he started doing the same thing. I thought maybe he had arthritis and it hurt to squat. I largely solved the problem by getting a litter box with high sides and a cover. He also sprayed sometimes elsewhere because of territorial issues with another male cat (both neutered) and it seemed he would shake his rear end when he was actually spraying. He didn't do this when he urinated in the box.
