Outside cats would definitely cause stress, if he hasn't seen a vet 'in years', now would be a good time, to rule out any physical reasons for him to avoid the litter box. Has there been any changes at all in his normal routines, or in his litter? To keep him off the counter you have to put something there until he learns not to, like tin foil, which they hate, or a clear plastic runner that they sell to put on the carpet. Turn it upside down to expose those little plastic 'nubs' , he won't step on those. I hope you find a solution, I know how frustrating it can be, I had a 'sprayer' that didn't stop for a year, but he finally calmed down from our move.  All the luck!

Yes, totally avoiding the box could be a sign of a Urinary Tract Infection.  Or it could be that he's stressed out and letting you know about it.

Along with the "keep offs" mentioned above, you can also put up up some empty soda cans or cans with a couple of pennies in them that he would knock over if he jumps on on the counter or table.  you would have to line the entire edge of where he jumps, but the sound of falling cans will usually be enough to startle them away.  I tried the plastic carpet runner (upside down), and it didn't deter my little guy at all.  Apparently he has tough feet

You can also try using a Calming Spray and/or Rescue Remedy to see if you can calm your boy down.  I am using it currently because we are doing a remodel, and it seems to work well.  Problem is, with outside cats roaming around, you would have to use it  ALL the time, until you can do something about those cats.  THAT is really your issue, it seems.  Do you know who they belong to?  We also have a couple of cats who seem to love our yard, and I find it quite annoying, because it drives OUR cats bonkers.  Nothing we can do about it though
At least my cats don't engage in improper urination. They just fight with each other alot because of it, which is also quite frustrating

Your replies are showing up here, so you are doing it right.

So from the sounds of it, the outdoor cats are not going anywhere.  WHY, I wonder, are they just now bothering your 11 year old, if they've been in your yard for quite some time?  I'm thinking maybe it IS a UTI afterall, since it doesn't sound like there is a new stressor in his life,or am I wrong about that?
