So far I have tried wood pellets and western family non clumping sand litter and my cat will pee or poop in it once or twice and stop using it once we clean the box. I've heard scary things about clumping litter such as the cat can get many health problems so we have been trying to stay away from it. It's hard to find something he likes. He just wants to go outside to poop.

There are various types of clumping litter, i.e., they're not all made of clay, and varying degrees of dust. Some are made of wood, walnut shells or the like. It's mainly young kittens you have to be careful with, since they've been known to eat litter, and clumping poses too much of a risk for them.

Some cats prefer non-clumping to clumping, or vice versa. There are two large groups of ferals at our shelter that surprisingly prefer clumping. The latest group was started out on soil, then slowly switched to non-clumping by mixing. Once a couple of boxes with clumping litter were added, they all stopped using the boxes with non-clumping, which surprised everybody.

We did that and he was using it the only problem was the dirt had bugs and many things inside of it and he caught an eye infection and he also had worms. Once we took the dirt out. He used the litter twice then stopped using it completely

Did you try gradually remove the dirt over time?

Well i suggest keep trying litter until you find one that works

You could try potting soil rather than actual dirt from outside, then gradually mix in increasing amounts of litter. Having two boxes, one with non-clumping, the other with clumping litter, would make it easier to find out his preference. Some cats require one box to urinate and one to defecate in. Some prefer open to covered boxes, or vice versa.
