My cat appears to have vision problems. I had the vet look at his eyes and the vet said his eyes appear normal. Despite this, I suspect he is far sighted. When playing, he can see distant objects but can't catch a string right in front of him.

He has random "freak outs". For example, a stuffed toy fell out of the closet and landed on my bed. He jumped on the bed later that day and completely freaked out. I thought he was thinking it was a strange cat!

I put him on the litter box every morning, as he sometimes has accidents. Yesterday I went to put him in the box, and he totally freaked out. He ripped me open with his claws and was absolutely panicked. Keep in mind we do this every morning! I hadn't turned on the light as I had just gotten up. I have no idea why he was so terrified other than he thought there was a ghost cat in that box.

My mom mentioned that he almost jumped on my other cat today. Apparently he didn't see her in the bed and nearly landed on top of her. She said he looked really shocked .

What you describe could be behavioral.  Why do you put him in the litter box, this shouldn't be necessary.  Even if vision impaired.  I have a 100% blind cat and she finds the box without trouble.

Cats also rely on smell and hearing to take stock of their surroundings.  Perhaps one of those senses has been affected?  How old is the cat?  Could senility be a factor?
