Teeth grinding, vomiting, pyloric stenosis, etc.

I need help! I have a persian cat (Gizmo). We think he is middle-aged, but not totally positive because he is a rescue. For the past 2 years, he has grinded his teeth, both when eating and also just randomly. The vet thought he had a bad tooth in October, so they removed a tooth. Unfortunately, this was of no relief. About 2 weeks later, gizmo started vomiting several times per day. The vet thought he had reflux (GERD), and we treated him with subQ Reglan and Pepcid for a few weeks. It stopped the vomiting, but missing one dose caused him to start again. In addition, the teeth grinding got worse and worse despite the meds. A second vet did an X-ray, barium swallow, and labs. Labs were normal, x-ray showed constipation, and barium swallow showed pyloric stenosis. He had surgery to repair the stenosis (pylorotomy). He also had an enema. He came home and did not vomit for 10 days. Then, it started again. Repeat x-ray revealed constipation was back, so he had another enema. Unfortunately, it didn't help. He is still vomiting a few times per day and the teeth grinding is worse than ever. He is still eating and drinking a little. His personality is relatively normal. I don't know what to do next. He has seen 3 vets with no relief of the grinding, vomiting, and constipation. Any suggestions or ideas or help is greatly appreciated! I'm desperate. Thank you!

Chronic constipation is a relatively common issue with kitties and what works for each kitty seems to differ some.  I think the teeth grinding and vomiting are related to the constipation.  Teeth grinding is something that many kitties do when they are nauseated which is related to the  constipation.  My kitty has IBD and he is prone to constipation. 

Many kitties are helped with a daily dose of Miralax.  Talk to your vet about it.  My vet had me start with 1/8 teaspoon daily and then adjust dose upwards until finding the dose that helped prevent the issue without causing diarrhea.  It's a tasteless powder that can be mixed in with wet food.

I haven't tried it but some people here have used an herb called Slippery Elm Bark which coats the digestive system.  If your kitty is on medication the medication needs to be given two hours separate from the SEB.  The coating that it does of the digestive system can effect absorption of medication.

Is your kitty on wet food?  Wet food often helps because it increases water in the diet.  

Fiber is kind of tricky.  A little extra fiber in the diet helps some kitties and makes things worse for others.  For now I would try the Miralax and/'or SEB and wet food to see if there is some improvement.

My kitty also has IBD with constipation. She gets Miralax twice a day in her wet food. Mine is also doing very well with the dry Royal Canin Fiber Response  but she only gets 1/8 cup of that a day. I know some are against any dry food but it has helped my cat.

Hope your kitty starts feeling better soon.

Thank you to everyone who has replied. Your suggestions are great and will be taken under consideration for sure! The problem is that Gozmo only eats a small small amount at a time...when he eats, he starts grinding his teeth so much that he can't even continue eating. He just looks miserable.

Maybe you could ask your vet about using Cerenia for the nausea while you work on getting a handle on the constipation.  It was developed for motion sickness in dogs but is effective for nausea in kitties.  I think it may still be off label use for kitties but there are several kitties here who have and are still taking it.  I really think that if the constipation issue is resolved than the nausea issue will also be resolved.
