Kitten Peeing?

Is it normal for a kitten around 3-4 months old to sometimes mess up and pee somewhere else other than the litter box? Blue has peed outside the litter box twice since we got her (that I know of.) I think the first time was because the litter box was too dirty for her standards... but I'm not sure if that's the same case this time. I scoop the box everyday (should I pick it up to twice a day since we have two kittens?) and I change it completely each week. This morning she pooped in the litter box.. then about a minute later she went across the room and peed on my jeans on the floor. Smokey has been using the litter box fine... and even used it right after she had just peed in the wrong place. Do different cats have different standards? Right now they stay in my room and there is one big litter box for both of them. Should I get another one? I'm using the litter that their parents family used - Johnny Cat. They also told me to feed them Purina Cat Chow for Cats of All Ages because that is what their vet told them. But Blue is always gassy and I was wondering if the food might be upsetting their stomachs since it's not kitten food?

Sorry this is so long... but I also have two more newbie questions...: to sanitize the litter box how much bleach/cleaner should be used? and does cat urine wash out of clothes easily? Thanks...

To play it safe, I would take her to the vet to see if she has a urinary tract infection. This is sometimes why cats pee out of their box. I would also get a second litter box as well.
