Cat is over-grooming groin area; need help ASAP!

Hello all,

Our male cat Mesut was hospitalized in March with a urinary blockage. He was catheterized and cleared, but they had to shave his lower groin in order to tape the tubes on. Ever since then he has obsessively over groomed that area. The hair never grew back and it was always red and rash-like (plaques, if you will). He has been to the vet about this issue more than six times since he left the hospital in March. He had two shots, steroids, medicated wipes, you name it- nothing worked. We finally put a cone on him but he figured out how to contort himself to still lick so we had to put an inflatable collar AND cone on it. That worked, we kept it on for a month and the hair grew back. We continued the medicated wipes to keep the healing process going as well.

Long story short- he's been MISERABLE. What cat wouldn't be? He couldn't groom at all, even through we wiped him down he still felt dirty because he kept trying to groom but couldn't. His personality went downhill, he acted depressed and it hurt us to see him like that. 

Tonight we finally said "enough" and took the cone/neck tube off. We put some anti-itch spray on, wiped him down, and let him go. It's been thirty minutes and he's already licked and bit so hard all the fur is gone again and it's red. He's been biting with such ferocity even when I go to make him stop I have to physically pull his head from his groin. It's like a magnet.

We are at a loss. We cannot keep taking him to the vet; we've already spent over $3,000 this year and seen no results. The thing is- even when he over grooms he never seems to be in pain, in fact he seems so happy! Playful and running around- but I know that's not good for him in the long run. We want to make it better we just can't afford to keep going to vets (yes, we've been to two different ones) and paying $$$$ for nothing. We could seriously run a cat pharmacy with all the medication he has had in the past six months.

Any advice? Any similar situations? I've heard it could be stress/allergies. He does not have fleas and is a fully indoor cat. We have hardwood floors with only one rug he rarely goes on. Any advice would be greatly appreciated!!!

Given the time that has passed I don't think this is an allergic reaction to something that was used on that area when he was treated.  Cats can become OCD and I am wondering if that is what is going on here.  I would ask the vet about treating it as if it's behavioral.  There are medications that can be given for it.  They wouldn't necessarily have to be permanent but to break this cycle that he is in.
