Should I be concerned? He never did that before. Box is kept clean and litter same and he does not have to share it.

I would say this is not a good sign.  It COULD just be a one time only, but typically it's a sign of a UTI, which I think you must suspect, based on your 2nd post.
What is best wet food for male cat with UTI?
There are really so many choices out there.  You would want something low carb, high protein.  Usually your pates are going to be lower in carbs.  Depending on your budget, you can even go with Friskies Special Diet, Fancy Feast Classics, Hounds and Gatos, Natures Variety Instinct, plus a whole lot of others.  Just try to stay away from too much fish.  And you need to make sure they get plenty of water, so if you add water to their food, that helps.  Plus try to keep their stress level to a minimum, if that's possible

Keep a close eye on him and make sure he pees. If he's blocked you have about 12 hours to get him to the vet.

Chicken flavored Weruva and Tiki Cat are supposed to be very good for UTI kitties. Weruva in particular is low in magnesium and phosphorus, even when compared to the prescription foods.

You might also try mixing extra water into his food.

Thanks raintyger, I make their food soupy by adding spring water. I just started that. Could spring water cause that? Normally they get tap water. He also threw up 2 days ago for first time in 4 years.

I was never really sure what was the best water to put in food. I choose not to have tap water because it does have more contaminants. You don't want to use distilled water. Not only will it taste nasty, but it can have negative effects with long term use. Mineral water I would shy away from, too, because it might have more minerals than desired. That leaves spring and the vague "drinking water." Spring water does have minerals in it, but you have to do some research to find out what minerals since it depends on which spring it's coming from. Drinking water you don't know where it comes from unless you read the label or do research; sometimes it is just tap water that has been filtered. Typically magnesium is added to any water that is filtered to add taste.

I was not sure it was pee it had no smell like pee. Today I found a teeny tiny wet spot on newspaper smaller than a dime at the edge of his litter box. I smelled it and it was not pee it was slimey like drool. Everything in his box looks normal. Pee and poo. Does this mean he is straining. I never see him, in his box, he does it all at night. Not sure which end the little bit of slime came from. It was clear small no odor. Does anyone out there have any ideas?

Hmmm.  Don't know about that drool issue.  One of mine sometimes throws up a little frothy saliva stuff after straining in the box IF it's been too long between meals.  It appears as you describe, only usually it's bigger than dime size and isn't right next to the litter box.  .

Another one of mine usually jumps out of the box before he's quite finished, thus leaving a trail of "dribbles" behind him.  Those wouldn't be slimey though, but would be smaller than a dime.

So...I really don't know what this might be .
But if he's peeing and pooping in the box, and acting fine, then seemingly things are ok.  When he threw up a couple of days ago, was it perhaps a hairball?  One of mine has been going a little nuts lately with the hairballs.

Obviously if you have any doubts at all, then take him in to be seen.  As long as he is actively peeing though, it's probably not an emergency. It's if they are blocked that it becomes emergent.
