For the first time ever at 4 years old and today I think he peed outside his clean box on newspaper for first time ever. Should I be worried he is very happy and energetic and eating well.

Throwing up very rarely is not usually a cause for concern. Throwing up is never normal, but when done very rarely is usually a sign of a one-time occurrence of something bad and not necessarily a lingering problem.
However, given the odd litterbox behavior in addition, I'd keep a close eye on him and err on the side of caution in taking him to a vet if you think something might be wrong. Good luck!
