My fat lazy cat needs to lose weight. HELP!

Well as the title says, my fat lazy cat needs to lose weight. She sleeps and eats all day. She's about 12ish years old, and doesn't even play. I was thinking of a can of fancy feast broken into 4 meals a day, with either a little lactose free milk or a tablespoon of dry food split into two between meal snacks. Any thoughts or suggestions?

I assume you mean a 3 ounce can of fancy feast since I'm unaware of bigger sizes.  Correct me if I am wrong.

That is not enough for ANY cat to eat in a day. Most cats should have around 5.5 or 6 ounces of wet food (1 large can) per day.

More importantly, you have to consider that 'rapid' weight loss is VERY unhealthy for cats. Your kitty should be losing 1-2% of body weight per week maximum.

It can help you figure out based on your cat's current and ideal weight how many calories she will need per day, and you can go from there.

That isn't enough calories for the cat and you'll risk hepatic lipidosis (sometimes fatal).  Here's a case that one of our members took on http://www.thecatsite.com/t/245236/mickey-my-27-lbs-foster-and-his-journey-back-to-a-healthy-weight with incredible success.  Several other members have used this method and helped their kitties lose weight safely.

It would help to know what the kitty eats now and how much.  Then more experienced members can help you with a plan.  Good on you for trying to get kitty to a healthy weight!
